Job Description:

Universal Healthcare Resources is seeking a highly qualified LPN/Licensed Practical Nurse to join our dynamic team in Bloomington, IN. In this role, you will play a pivotal part in ensuring the health and well-being of employees within various occupational settings. Position Information:

The duties of a Licensed Practical Nurse include assisting patients with daily activities, monitoring vital signs, aiding in physical therapy, providing emotional support, ensuring a safe environment, and maintaining accurate patient records.

Responsibilities:

• Assess patients' physical and mental health status upon admission to a rehabilitation unit.
• Develop and implement individualized care plans in collaboration with the healthcare team.
• Administer medications, treatments, and therapies as prescribed by physicians.
• Monitor and evaluate patients' progress towards rehabilitation goals.
• Educate patients and their families about medications, treatments, and self-care techniques.
• Coordinate with other healthcare professionals, such as physical therapists and social workers, to ensure comprehensive care.
• Document patient assessments, interventions, and outcomes accurately and timely.
• Provide emotional support and encouragement to patients during their rehabilitation journey.
• Assist with discharge planning and transition of care to ensure continuity of services post-rehabilitation.
• Adhere to ethical and legal standards of nursing practice and maintain a safe and therapeutic environment for patients.

Requirements of LPN/Licensed Practical Nurse

• Minimum 1 year of LPN experience required.
• Current active license as a LPN
