ORGANIZATIONAL SUMMARY
- The Licensed Practical Nurse reports to the Administrator/Clinical Manager/RN Supervisor.
- The Licensed Practical Nurse is responsible for maintaining communication between members of the interdisciplinary team, including the client, client representative (if any), family members, nursing staff, and attending physicians and other care providers.
PRINCIPLE RESPONSIBILITIES
- The Licensed Practical Nurse (LPN) will assist the registered nurse in performing certain nursing procedures in accordance with the Plan of Care established by the physician and Administrator/Clinical Manager/RN Supervisor.
- Evaluate clients’ needs, review all medication, and involve other disciplines as appropriate.
- Provide specific skilled procedures as assigned and in accordance with acceptable standards for the LPN, including sterile dressing changes, administration of oral, IM, and rectal medication, routine ostomy care, catheter care, observation of signs/symptoms, tube feeding, teaching client, client representative (if any), and family in self-care techniques.
- Reports adverse signs and symptoms to the supervising RN and progress toward achieving client goals.
- Responsible for monitoring for and reporting any suspected abuse to the supervisor immediately
- Prepare current and accurate records showing treatments, teaching and training, observations, and client responses and reports above to Administrator/Clinical Manager/RN Supervisor. Abides by the Agency’s Documentation of Services policy.
- The Licensed Practical Nurse (LPN) provides client services as assigned and is directly supervised by the Administrator/Clinical Manager/RN Supervisor or Registered Nurse designee.
- Communicates and verifies all new physician orders with Clinical Manager, RN Supervisor or Administrator prior to implementation.
- Responsible for participating in QAPI activities as requested by Administrator/Clinical Manager/RN Supervisor.

Mon - Fri, 3:30 - 6:30pm
Saturdays, 8:00 - 4:30pm
Summer Hours:
Mon - Sat (no Wednesdays), 8:00 - 6:30pm
Salary : $47,400 - $60,000
