Job Description
M-F 8a-5p with possible evenings and Saturdays to attend occasional community events or health fairs. Role requires travel within Prince George county.
I. General Summary
Under general supervision helps develop, plan, manage, and evaluate community health improvement and outreach programs, including health screenings, disease prevention programs, and other programs in support of the community health initiatives of the local organization and University of Maryland Medical Systems.
II. Principal Responsibilities and Tasks
The following statements are intended to describe the general nature and level of work being performed by people assigned to this classification. They are not to be construed as a detailed description of all the work requirements of the position.
- Works with departmental leadership to develop and maintain CBISA platform, materials and instruments to be used to accurately collect community benefit data. Experience with community benefits reports and community health needs assessments reports.
- Develops and manages data reports in CBISA for regulatory reporting, including but not limited to community benefits reports and community health needs assessments.
- Responsible for inputting Community Health Improvement data indicators and metrics into Cascade, a strategy and execution platform, allowing for system-wide sharing of the organization's strategic plan and goals.
- Develops & manages data capturing protocols and processes for the collection of community benefit initiatives. Monitors and implements a quality assurance process and provides technical assistance in the use of CBISA for staff.
- Provides supervision and management to the community health specialist and community health workers; providing oversight for projects and program workflows.
- Initiates, coordinates and enforces protocols and systems among staff to increase program and project coordination.
- Manages health screenings and outreach events in the community and system hospital campuses by partnering with professional staff, business, identified targeted communities and community-based organizations, key stakeholders, and agencies to facilitate and coordinate efforts.
- Oversees a variety of health promotion and disease prevention programs with managers and outside vendors, contractors and other key stakeholders to provide programs to the community.
- Communicating with local nurses and doctors to provide better community support.
- Initiates and coordinates procurement and distribution of community health promotional materials.
- Prepares detailed reports outlining costs and future community events and promotions. Prepares correspondence and reports outlining costs and future community events and promotions.
- Coordinates and manages staffing to safely deploy health programs in the community.
- Represents local organization and UMMS at community health events as assigned.
- Represents local organization and UMMS on appropriate health coalitions, local health department, and community organizations’ work groups and committees.
- Serves as class facilitator for prevention-based courses offered to the public and patients.
- Serves as the liaison between the local organization administration and UMMS on community outreach and benefit activities and programs.
- Follows all internal and external policies and regulatory requirements to deliver safe, high quality programs.
- Other duties as assigned.

III. Education and Experience
- B.S. Degree (e.g.,Healthcare Management, Health Education, Health Promotion or related field) or at least 4 years of experience required
- Master’s Degree preferred.
- 2 years of people leadership experience with having direct reports required.
- At least 2 years of experience in health care environment is required (preferrably in a hospital setting).
- Working knowledge of computer applications with experience in word processing and spreadsheet software such as Microsoft Word, PowerPoint, Excel, and CBISA.
- Requires excellent communication, interpersonal and organizational skills.
- Demonstrated ability to work effectively with the public, community partners, vendors, and professional staff.
IV. Knowledge, Skills and Abilities
- Weekend, shift work, holiday, on-call, and overtime may be required to attend community events or health fairs.
- Work is often performed outside of the hospital in various locations throughout the Baltimore metropolitan area. Locations may include local church halls, schools, senior centers, parks, mobile van, or business offices.
- Significant local travel required.
