We are seeking LPNs to join our interdisciplinary team providing skilled nursing services to clients in their home. We offer an intensive mentoring program upon hire and ongoing education. The Adult Home Health team delivers care to patients in the community, providing a wide range of skilled nursing care. We are a comprehensive team comprised of RNs, Physical Therapists, LPNs, LNAs, SLPs, Social Workers and administrative support, all collaborating together to support patients to remain in living in the setting of their choice, their home.
The Community Health Nurse, LPN will be responsible for the delivery of basic nursing care to patients in a home care settings, under minimal clinical supervision, as directed by a Registered Nurse, and in accordance with an established nursing plan of care.
The general territory for this position is Burlington, Winooski and South Burlington. At times travel may be required outside of assigned territory but a general service area is assigned and part of typical travel. All mileage is reimbursed at the federal gas rate (.65c/mi)!
Qualifications:
- Accredited School of Nursing LPN Program
- Two or more years of directly related home/public health, hospice and/or maternal/child health nursing experience
- Current LPN licensure or compact licensure recognized by the State of Vermont required
- Previous home care desired.
