The Population Health Services Organization (PHSO) Care Management team at UVM Health Network is hiring for a Manager of Care Management!
This role will be centered on providing support to our Pediatric and Family Medicine teams.
WHO WE ARE:
The UVMHN (University of Vermont Health Network) PHSO (Population Health Services Organization) is a physician and payer partnered operation that provides the right constellation of services (e.g. care management and care coordination, data and analytics, provider reporting, Quality, Risk Adjustment, and Value-based Contract Performance, etc.), that enables the health network to successfully perform in High Value Care and Value-Based Contracts.
JOB DESCRIPTION:
Manager of Care Management leads interdisciplinary core teams in delivering comprehensive, consistent, timely, effective, and patient/family centric care. The Manager is responsible for the oversight of the staff and daily operations for the PHSO Care Management department. Provides training, support to direct reports, encouraging individual and team development. Participates in process improvement: ensuring policies, procedures, and customer-service are constantly monitored, evaluated, and supported. Assist the Director in ensuring regulatory compliance. As part of the management team, the manager assists the director in fulfilling departmental goals.
EDUCATION:
Current RN licensure or compact licensure recognized by the State of Vermont is required. The University of Vermont Health Network is moving toward an all Bachelor of Science in Nursing (BSN) workforce. Current internal RNs are strongly encouraged to pursue a BSN to support this initiative.
OR
Master's in social work from an accredited school of social work. LICSW required.
Case management accreditation by a nationally recognized accrediting body for case management (examples: CCM, ACM or ANCC certifications) preferred.
EXPERIENCE:
3-5 years of leadership experience in a healthcare setting required
3-5 years of clinical experience in a healthcare setting (or equivalent) required
Care Management/Care Coordination experience required
