This is a full time, 1.0 FTE day shift position. Hours will be primarily Mondays Fridays, 8:00am 4:30 pm, no weekends required. Hours may vary based on the operational needs of the department.

We are seeking an Ambulatory Social Worker to:
Evaluate complex patient and family situations which have legal, ethical and psychosocial components.
Collaboratively develops patient goals, monitors progress, addresses barriers to goal achievement and evaluates outcomes of individualized care plans.
Engage patient/family in the treatment and decision-making process with respect to patient rights, principles of confidentiality, respect for patient privacy, and right to self-determination.
Utilizes crisis intervention, conflict resolution, and mediation to address patient/family concerns.

Education :
Minimum Masters Degree in Social Work
Work Experience :
Preferred Two (2) years prior hospital based social work experience.
Licenses and Certifications :
Minimum Licensed in the state of Wisconsin with one of the following within 90 days of hire:
Certified Advanced Practice Social Worker (CAPSW)
Certified Independent Social Worker (CISW)
Licensed Clinical Social Worker (LCSW)
