Work Schedule :
100 % FTE, 40 hours/week. 8 or 10 -hour day/evening shifts scheduled Monday, Tuesday and Friday: 7:00 AM 4:30 PM, Wednesday: 8:00 AM 12:00 PM and Thursday: 9:00 AM 6:30 PM. Hours may vary based on the operational needs of department.

We are seeking a Physical Therapist to:
Evaluate and treat an orthopedic/spine chronic pain patient population of all ages, develop and guide this patient population through appropriate individualized rehabilitation programs.
Efficiently communicate with physician referral sources, and other UW Health departments for care coordination and collaboration.
Work within an interdisciplinary pain management team to continually provide exceptional patient and family centered care.

Education :
Minimum Bachelors degree in Physical Therapy is required.
Work Experience :
Preferred Two (2) years prior relevant experience.
Licenses and Certifications :
Minimum Licensure as a Physical Therapist in the State of Wisconsin is required. Certification in Healthcare Provider Cardiopulmonary Resuscitation and Automatic External Defibrillator is required within three months of hire.
