Unlimited Care Inc. is looking for a Service Coordinator Assistant for the Medicaid Waiver Participants in the Nursing Home Transition and Diversion and Traumatic Brain Injury program in Brewster, NY.

Essential Job Functions:
• Assist Service Coordinators in developing comprehensive Service Plans, provision of services, and scheduling participant medical visits and transportation requests
• Handle phone inquiries, email and fax correspondence and take necessary steps to forward messages or remediate service issues as they arise
• Maintain, organize, and assure appropriate filing of all charts and reports in a timely manner, including annual thinning of all charts
• Attend Team Meetings and service plan development meetings, as appropriate

Candidates must possess:

· A bachelor’s degree in social work, psychology, or closely related field

· Proficiency with Microsoft Suite

· Team player able to work both independently and in a group

· Possess strong verbal and written communication skills

· Ability to work effectively in a fast paced, multi-tasked environment

· A valid NYS driver's license is required and ability to travel to other UCI branches
