Hospice House Supervisor (Collins House)

Purpose:
An on-site position that is accountable and responsible for the overall quality of service, overseeing the planning, developing, organizing, and integrating, of clinical care and coordination of the Collins House residents.

Responsibilities:

• Ensures patient care is delivered according to established care guidelines.
• Plans, directs, and participates in the delivery of client care services, and scheduling activities.
• Recommends, implements and interprets policies and procedures relevant to the provision of patient care services.
• Communicates effectively with assigned staff, to monitor the changing needs of the patient and family.
• Leads regularly scheduled Staff meetings.
• Participates in the hiring/orientation of qualified employees to meet patient care needs.
• Supervises and monitors productivity expectations for assigned staff, ensuring coverage of services.
• Provides personnel management functions of all assigned staff, including evaluation of performance and implementation of disciplinary actions as appropriate.
• Facilitates patient care coordination and operations including organization and management within established policies, procedures, standards of care, budgetary parameters, and strategic objectives.
• Accountable for effective performance and follow-through of all assigned responsibilities and for completing responsibilities within designated (or agreed upon) timeframes.
• Completes responsibilities in a manner consistent with organizational policy, goals, and values.
• Supports teamwork and collaboration among colleagues, staff, and other stakeholders to ensure efficient and effective strategic plan implementation and success.
• Provide leadership and promote coordination of care through communication with members of the multidisciplinary team such as the hospice interdisciplinary team, social services, dietitians, spiritual counselors, and therapists, to meet the complex needs of the terminally ill patient.
• Responsible for providing monthly management reports.
• Document services in appropriate records and documents.
