As an RN with UPMC Home Healthcare, you’ll collaborate with our team of specialized nurses, rehabilitation therapists, social workers, dieticians, aides, and trained administrative staff to provide personalized patient care in a home setting.
Responsibilities:
- Coordinates case management activities within an interdisciplinary health care team to ensure appropriate delivery of services to patient.
- Attends compliance training and adheres to the organization standards of conduct, policies, and procedures.
- Focuses on customer service and continually strives to perform the duties of their job in a manner that will result in optimal patient satisfaction.
- Completes an initial and comprehensive assessment of patient.
- Constructively receives feedback and direction.
- Takes action to improve knowledge, skills and performance based on feedback or on self-identified developmental needs.
- Initiates, coordinates and ensures ongoing communication with the physician and other members of the interdisciplinary health care team, including evaluating patients and family centered health teaching.
- Provides direct patient care, evaluates outcomes, and adjusts nursing care process as indicated to ensure optimal patient care.
- Establishes the plan of care in collaboration with the physician, patient, and interdisciplinary team members in accordance with the physician orders and patient needs.
- Assigns other interdisciplinary team members as appropriate to assist in the appropriate delivery of services as ordered on the plan of care to the patient.
- Supervises and teaches other nursing personnel including LPNs and Home Health Aides as needed.
- Attends and participates in case conferences, team meetings, and other work-related meetings.
- Recognizes limitations and requests assistance and direction to meet performance goals.
- Completes all necessary documentation which may include visit assessment, plan of treatment, and verbal orders accurately and promptly in electronic documentation system while in the patient's home and within the agencies and regulatory standards.
- Data syncs (transmits) information same day.
- Identifies, investigates, and recommends solutions to organization issues.
- Exercises independent judgement in matters concerning emergent and non-emergent patient care needs and communicates with the physician.
- Identifies, develops and participates in process improvement opportunities within the agency that will enhance the quality of the services we provide.
- Participates in weekend and on-call coverage rotation for the agency as per BU guidelines.
- Demonstrates knowledge and understanding of the UPMC standard of care delivery and proficiency in all aspects of patient care, to include complex and/or specialized care.
