The Upper Cumberland Development District / Area Agency on Aging and Disability is accepting applications for an Options Counselor. The Options Counselor assists individuals in making informed decisions about long-term care support with a focus on home and community-based services (HCBS). The Options Counselor completes in-home assessments to assess needs and determine eligibility for HCBS programs, develops and manages care plans, coordinates HCBS services with subcontracted providers and connects individuals with other community resources. The caseload for this position includes clients residing in the following counties: Clay, Overton, Pickett and Putnam.
Qualifications include:
· Excellent computer skills
· Ability to effectively multitask
· Bachelor's Degree in social work, psychology, gerontology, sociology, counseling, nursing, or equivalent degree; or a minimum of completion of 2 years attendance at an accredited college or university and a minimum of 2 years experience in the field of social work or a related field. Additional related experience may be substituted for education.
