ESSENTIAL RESPONSIBILITIES:
Activities include but are not limited to the following:
- Provides initial and/or ongoing service coordination across Oneida County.
- Ensures all services are established and maintained for the families.
- Assures supports are in place inclusive of service provider and family contacts.
- Collaborates with all service providers and establishes team communication plan.
- Monitors goals on a continuing basis.
- Monitors quality of team interactions.
- Advocates for additional services and linkages as appropriate.
- Provides written documentation of services.
- Maintains monthly contact with families.
- Develops a transition plan that will include the steps needed to help the family leave the EIP and begin new services, if eligible.
- Submits complete billing documentation in a timely manner.
- Participates in NY State regulatory training as required.
- Provides semi-annual and annual case reviews.
- Participates as a member of the interdisciplinary team.
- Ensures compliance with all pertinent government and agency regulations and operating standards.
EDUCATIONAL REQUIREMENTS/ QUALIFICATIONS/ EXPERIENCE:
- A minimum of one of the following educational or service coordination experience credentials:
i. two years experience in service coordination activities as delineated in this Subpart (voluntary or part-time experience which can be verified will be accepted on a pro rata basis); or,
ii. one year of service coordination experience and an additional year of experience in a service setting with infants and toddlers with developmental delays or disabilities; or,
iii. one year of service coordination experience and an Associates degree in a health or human service field; or
iv. a bachelors degree in a health or human service field; or
v. a license, certification, or registration in one of the professions listed in section 69-4.1(al) of this Subpart.
OTHER REQIREMENTS:
- Must have or obtain the NYS Early Intervention Coordination Certification within the first 3-months in this position.
- Must complete 40 hours of specialized/designated training within the first year in this role per regulatory requirements.
- Travel is required. Must have a valid NYS Driver’s License.
- Intermediate computer skills (Including all Windows program, Outlook, Adobe Acrobat, Internet Browser, Access, Word and Excel).
- This position has regular and substantial unsupervised or unrestricted physical contact with individuals receiving services and is required by law to be fingerprinted for a criminal history record check under OCFS (Office of Children and Family Services).
- A valid NYS driver’s license is required
