Job Title: Nurse Navigator-Pharm for Advanced Prostate Cancer

Reports to: Chief Operating Officer

Position Summary:

The Nurse Navigator for Advanced Prostate Cancer will work collaboratively with the Urology of St. Louis (USL) team to assess, plan, facilitate, track, and monitor patients with prostate cancer. The Nurse Navigator for Advanced Prostate Cancer works closely with the patient and partner; makes appropriate referrals to meet the patient’s individual, cultural and financial needs, and monitors and evaluates services provided. The Nurse Navigator for Advanced Prostate Cancer works as a liaison to and in collaboration with patient, physicians, and other healthcare providers.

· Assist patients in understanding their diagnosis, treatment options, and the resources available. Including reinforcing education to patient on available resources, distributing education materials for patient use

· Serve as patient navigator and primary contact for prostate cancer patients

· Assist with the implementation of USL advanced prostate cancer protocols

· Participate in prostate cancer disease planning and help ensure compliance

· May assist in development of patient education materials, clinical pathways, quality studies, and clinical trials

· Coordinate education of patients and partners on the function and proper use of oral Oncolytics and Provenge. Including hands-on assistance of patients with use of medications, infusion enrollment, infusion day support, and education of USL staff on APC protocols.

· Serve as an essential link between patients and all other care providers. Facilitate appointments for consults and support services, including medical/radiation oncologist, hematology/oncology, cardiology, physical therapist, outpatient radiology services, primary care, and palliative/hospice services

· Work with data analytics program to mine data from EMR and ensure adherence to appropriate pathways and follow up (Provenge, Xofigo, Orgovyx, Xgeva)

· Work with data analytics program to manage critical Advanced Prostate Cancer Patients and ensure adherence to care plan as outlined by APC provider.

· Identify benefits and coordinate resources based upon patient’s needs, insurance coverage and community programs

· Obtain authorization, appeals, and facilitate peer to peer reviews for patients and providers as needed

· Follow through with patient’s medication treatment plan, ensuring they have received education and medication as prescribed

· Identify barriers to the plan of care and intervene as appropriate to provide optimal patient care

· Maintain professional relationships with pharmaceutical representatives, and other outside resources

· Assist in preparation of urology case presentations to the Board of Directors

Education and Experience

· Registered Nurse in the state of Missouri with a minimum of 3 years clinical experience, preferably in urology / prostate focus

· Current Basic Life Support (BLS) certification required

· Patient navigation experience desired
