Social Services Coordinator

Job Summary:
Coordinates input from the team and provider practices to provide continuity of care across the care continuum for a defined
population.
Qualifications:
Education – Required
- Bachelor's Degree in Social Work, Sociology, Psychology or related field.
Experience – Required
- 4 years Medical or clinical setting.
Additional Qualifications
- Training in clinical setting (hospital and/or ambulatory) performing social services
Essential Functions:
- Assists practices to identify community resources in a defined geographic area.
- Assesses social determinants of health needs and coordinates with other social services/agencies to ensure patient needs are met.
- Complete financial assistance applications with patients (ex. Medicaid, Sliding Fee Scale, and other benefit programs)
- Documents all interventions succinctly and in a timely fashion in the medical record.
- Documents patient/family understanding of barriers to appropriate care and transition planning.
- Provides input and participates in the development of the patient-centered multidisciplinary care management plan.
- Evaluates ongoing effectiveness in meeting established care goals and revises plan, as needed.
- Provides telephonic or face-to-face outreach to patients based on referrals from the team and provider practices.
