What You Will Do:
The Community Health Worker (CHW) is a frontline public health worker who is trusted by, and has a close understanding of, the community served. Such relationship enables the CHW to serve as a liaison between health and social services and the community, in order to facilitate access to services and improve the quality-of-service delivery. This position may include some weekend or evening work.
Primary Responsibilities and Duties
Advises on and develops effective marketing and outreach materials. Conducts neighborhood canvassing, street outreach, and outreach in public settings. Provides education and materials to key influencers, such as barbers/hair stylists, coaches, fraternity members, pastors, neighborhood leaders, front-line workers, etc. Partners with community organizations and key stakeholders to identify and recruit program participants. Identifies and recommends opportunities for new community or agency relationships. Provides direct assistance to program participants, which includes, but is not limited to, health education/coaching, medical appointments/logistics, patient advocacy, basic health screening/monitoring, and access to food/housing resources. Screens individuals for health-related social needs & primary care access. Conducts follow-up with each program participant at least once every three months and provides direct assistance as needed. Refers program participants to useful and meaningful medical and supportive services vetted through site visits and client feedback. Collaborates with team to address successes and challenges in program implementation, relevant current events, community feedback, resources, etc. Maintains a database of program participants, participant encounters and progress toward established goals. Responsible for reporting data monthly. Collaborates with internal team members and external partners on goal setting. Conducts phone-based outreach. Delivers presentations to consumer and resident groups in person and via social media. Advises on and develops visuals for collateral materials on all social media platforms. Participates in CHW training as assigned and required.
What You Will Bring:
Education
High School Diploma or GED required
Associate degree in health or human services or related field preferred
Course training in community health work preferred
Experience
Minimum two years of work experience or Associate degree accepted in lieu of work experience, required
Health or human services experience preferred
Experience in a community health or similar role preferred
Certification & Licensures
Valid driver's license and reliable transportation required
Qualifications
Excellent interpersonal skills and ability to interact professionally and gain trust with culturally diverse individuals is required
Familiarity with regional and local landscape required
Prior experience working with community resources and social services in focus region preferred
Ability to show empathy and to handle confidential information with discretion and professionalism is required
Computer skills and proficiency in the use of handheld tablets required
