What You Will Do:
Facilitates safe and timely discharge of patients requiring post acute care services, or special transportation in coordination with care team. Coordinates the legal process and paperwork involved with: involuntary patient admissions, protective services, guardianships and adoptions.
What You Will Bring:
Education
BSW or MSW from an accredited social work program or equivalent education/experience required
Experience
Previous social work experience in a medical setting preferred
Qualifications
Knowledgeable in stages of human growth and development for neonate, pediatric, adult and geriatric populations required Skills in interpersonal relationships, clinical assessment, group process and high levels of verbal and written communication required Ability to interact with other professionals as part of a multidisciplinary team required
