Summary of position:
The HCC Project Manager is responsible for coordination and supporting of all projects related to the Risk Adjustment program within the organization. Lead interaction with providers and staff regularly. Training and maintaining up to date program requirements. Accountable for facilitating best working relationship between the assigned network and the physician trainer.
Essential Duties and Responsibilities include the following:
- Coordinate all activities for Risk Adjustment initiatives.
- Prioritize, distribute, track, and develop action plans to ensure timely completion of special projects.
- Work with the physician offices as applicable to meet Risk Adjustment guidelines per regulations
- Be sure to adequately train and manage provider staff, related to keeping them current with the necessary tools to practice within the guidelines of our programs.
- Provide administrative support to fulfil all Risk Adjustment needs pertaining to the provider offices for improving overall metrics.
- Train and educate provider offices how to follow-up and follow-through with completed patient visits, documentation, on the Health Risk Assessments.
- Communicate closely to maintain professional relations throughout this process.
- Work directly with the Risk Adjustment team to meet specified goals as determined by the executive leadership.
- Provide on-going meetings and education sessions as needed.
- Work to develop and manage a project plan.
- Develop departmental and individual goals.
- Perform other related duties as assigned or requested.
The pay range for this position at commencement of employment is expected to be between $66,726.40-$70,000/year; however, base pay offered may vary depending on multiple individualized factors, including market location, job-related knowledge, licensure, skills, and experience.
The total compensation package for this position may also include other elements, including a sign-on bonus and discretionary awards in addition to a full range of medical, financial, and/or other benefits (including 401(k) eligibility and various paid time off benefits, such as vacation, sick time, and parental leave), dependent on the position offered.
