Job Summary:
This Clinical Case Manager position is responsible for providing services to clients with mental and behavioral health needs on an individual and group basis, including creating treatment and discharge plans for each client assigned to their caseload. This is a full time in- office position.
Job Duties and Responsibilities:
Core duties and responsibilities include but are not limited to the following. Other duties may be assigned as necessary.
- Process intakes, discharges, and treatment plans with a caseload of up to 15 clients
- Ensure client information is documented, accurate and updated in EMR.
- Ensure client records for assigned case load are comprehensive, complete, neat and meet licensing standards.
- Complete Clinical assessment and treatment plans for all clients
- Initiate discharge planning upon admission for assigned clients, and update discharge plan weekly.
- Completes discharge audits for each assigned client upon discharge
- Facilitate comprehensive evaluation and diagnostic summary for assigned clients
- Facilitate multidisciplinary treatment planning and updates for assigned clients
- Facilitate individual sessions for all assigned clients twice weekly
- Facilitate group counseling, psycho-education, and meeting per program schedule
- Ensure and encouraging clients to attend group on time
- Family/ conjoin therapy as specified in individual clients treatment plans
- Complete all progress notes for individual and group sessions within 24 hours
- Ensure client compliance with UA policy
- Attends and participates in weekly clinical team meetings
- Call and update families on progress of treatment in accordance with all HIPAA laws
- Perform other duties as assigned
Required
- Registered Behavioral Technician (RBT) Certification
- Current CPR & First Aid Certification
Required Knowledge, Skills, and Abilities:
- Must demonstrate ability to provide appropriate care based on client individual needs, including the age demographics of Valor’s client population.
- Knowledge of principles, practices, techniques, and trends in alcohol and drug counseling.
- Theoretical basis and practical experience with crisis intervention
Job Type: Full-time
Pay: $18.00 - $21.00 per hour
