Valor HospiceCare is looking for a Full-time Registered Nurse RN for the Green Valley area. The Registered Nurse is a member of the interdisciplinary team, and reports to Clinical Coordinator for the provision of skilled nursing assessment and hospice care in order to maximize the comfort and health of patients and families consistent with Valor HospiceCare policies and procedures.

- Responsible for providing skilled nursing, intervention, supportive care and admissions in patient homes in accordance with the Hospice Interdisciplinary plan of care for each patient
- Provide liaison/information visits as assigned
- Serve as a Valor HospiceCare staff liaison with physicians, RN case managers, hospital/institutional nurses, social workers, discharge planners and community agencies
- Visit patients and confer with hospital staff to ensure continuity and control of the plan of care
- Initiate communication with attending physicians, other hospice staff members and other agencies as needed to coordinate optimal care and use of resources for the patient/family
- Maintain regular communication with the Nurse Case Manager, Program Director, and Medical Director
- Occasional travel to satellite locations may be required
Requirements
- Possession of current license as a RN in the state of Arizona
- Nursing experience in the acute or home health settings
- Demonstrated ability to educate, assess and respond to the needs of patients and families in varied settings.
- Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order
- Possesses and maintains current CPR certification
- Demonstrated skill in nursing practice, ability to cope with family emotional stress and tolerance of individual lifestyles
- One year hospice experience preferred
- Bilingual in English and Spanish preferred
