Vanna is reimagining care and community for people living with serious mental illness (SMI).
The current system is failing individuals with SMI, resulting in a high risk of loneliness (2.3x), homelessness (6x), unemployment (30x), incarceration (25x), and reduced life expectancy (by 20 years). Vanna believes that no person is too ill to recover and that the solution is social, not just medical. We are looking for a Vanna Coach who is passionate about working in and supporting their community. A Vanna Coach is exceptional at advocating for individuals living with serious mental illness throughout recovery, including during hospital/ED discharge, housing transitions, and day to day support.
Coaches will play a critical role in physical and behavioral health coaching, and care navigation when required for Vanna members. The partnership with members starts with building a trusting relationship, understanding their personal goals, and connecting them to the best resources to achieve those goals. Vanna Coaches will work alongside a Team Lead, clinicians, and other specialists, and through their community team will have access to the support that they need to empower Vanna members to flourish. This role is for Coaches to bring their lived experience with recovery to support Vanna members through their personal recovery journey.
Vanna Coaches will help develop an innovative, recovery based program, and we deeply value the insight that you will bring to the team. Vanna will leverage our purpose-built, mobile-first technology to redefine what it means to work in community health.
Responsibilities
- Conduct community outreach to individuals living with serious mental illness, at locations including homes, community events, ERs, Crisis Response Centers, shelters, and other frequented locations
- Engage individuals using a peer-centered approach based on mutual respect, and invest time to build a trusting relationship with each individual
- Identify immediate social, behavioral, and physical health needs of individuals
- Help individuals connect to resources to address immediate social, behavioral, and physical health needs
- Learn about the individual’s personal goals
- Provide critical insights into an individual’s goals, needs, preferences, and background with team to create holistic, person-centered engagement strategies with the Community Team
- Promote recovery-oriented thinking
- Foster relationship building between the individual and the rest of the Community Team
- Act as role models for individuals and provide encouragement across all points in recovery
- Assist in coordinating care for individuals when required
- Attend appointments and help coordinate transportation for members when requested
- Advocate for the individual throughout their recovery journey
- Provide physical health coaching (Vanna will provide training) where applicable
- Identify opportunities to connect individuals to existing community events, local clubhouses, or other individuals that have similar interests
- Adhere to documentation protocols for outreach, encounters, and other requirements related to responsibilities
- Provide input to the team to make sure that Vanna’s model is peer-led, person-centric, and adapted to local needs and culture
- Provide feedback to Vanna technology and operations teams on existing processes and tools
- Lead or co-lead classes and trainings
- Provide employment coaching and support
- As necessary, address crisis intervention and other urgent/emergent situations to promote the safety and well being of members
About You
Need to Have
- 2 years of experience working directly with individuals with serious mental illness utilizing motivational interview techniques
- Lived experience in the mental health space
- Experience working in and around the community of Philadelphia, PA
- Belief that individuals suffering from serious mental illnesses can and will recover to achieve life goals when provided the opportunity, support, and community
- Passion for supporting your community
- Are able to work in a team
- Are willing to learn how to work with Google, Slack, and Vanna EHR tools
- Basic computer and phone skills
- A valid driver’s license
- High school diploma or equivalent
Nice to Have
- Certified Peer Specialist or Community Health Worker Certification
- If you don’t have either certification currently Vanna will provide you with the resources to complete a certification course within your first year of employment.
- Access to an operational and insured vehicle and willingness to use it to transport person served
- Bilingual in Spanish and English to ensure we are meeting all members in their most comfortable language
