Community Health Worker (CHW) must be dedicated and compassionate individual who will work with clinical and health center staff to engage and advocate for Medicare and Medicaid beneficiaries, address social determinant of health needs, provide health education, and help beneficiaries complete health risk, SDOH, mental health, and safety assessments. The CHW plays a crucial role in bridging the gap between healthcare services and community resources and service supports, focusing on improving health outcomes and promoting health equity. This position requires some travel. Must reside in Arizona.
Essential Job Duties
Engages the community to build strong relationships within the community organizations and resources to knit together a social support network for Medicare and Medicaid beneficiaries.
Act as a liaison between community members and healthcare providers.
Provides health education to Medicare and Medicaid beneficiaries and their care givers to raise health literacy and emphasize preventive care and healthy lifestyle choices.
Conducts outreach programs to raise awareness about key health issues.
Utilizes information and IT analytics to identify Social Determinants of Health (SDOH) challenges and identify gaps in resources and specific SDOH challenges impacting target areas served by health centers.
Collaborates with healthcare professionals to address SDOH barriers.
Coordinates referral and facilitates the referral process for Medicare and Medicaid beneficiaries and assists them to access necessary healthcare services and resources to ensure seamless referral process.
Responsible for data collection and reporting, conducting in-person non-clinical assessments to understand care needs, preferences, and socioeconomic barriers.
Advocates for Medicare and Medicaid beneficiary needs.
Must Maintain accurate and detailed records for reporting purposes.
Assist with the development of community health worker training programs.
Performs home visits to home bound beneficiaries.
Qualifications & Experience
Associates degree or equivalent (Bachelor's degree preferred).
Minimum of three years’ experience working in a hospital, clinic, or health center as medical assistant, patient attendant, social worker, or nursing assistant.
Certification as Community Health Worker required.
Bi-lingual in Spanish desirable.
Must have Arizona Driver License
Must have knowledge of Southern Arizona Communities, social services and community resources.
Previous experience in community outreach
Strong communication and interpersonal skills.
Cultural competence and ability to work with diverse populations.
Knowledge of medical terminology.
Must have training in basic first aid and CPR.
Experience with Microsoft word, excel, and power point.
Position requires 24 hours of continuous education every two years.
