What are the responsibilities and job description for the Outreach Coordinator position at Ventura County Adult Day Health Care?
Ventura County Adult Day Health Care is a community-based program providing a variety of medical, therapeutic, rehabilitative and recreational programs and services for adults 18 and older. We provide a safe and secure environment for adults to spend time under the cared supervision of experienced staff.
Our mission is to help prevent premature institutionalization by offering stimulating and enjoyable group experiences for participants to maintain their independence and also to provide respite for family members and caregivers.
Outreach Coordinator position responsibilities:
1. Positively represent VCADHC in contacts with all Healthcare, Human Service, Community Organizations, and individuals.
2. Continually seek and research new sources of prospective participants in the community.
3. Grow and maintain a comprehensive list of referral sources.
4. Network with local referral sources to encourage referrals and ensure a smooth referral process.
5. Under the supervision of the Program Director the Community Outreach Coordinator shall perform assigned tasks involving:
- Make regular calls and in-person visits to potential referral sources.
- Coordinate with the Social Work department to review the list of incoming referrals.
- Assist Activity Department in planning and developing activities for participants.
- Coordinate VCADHC’s Social media accounts and web presence.
- Attend staff meetings and in-services and educating training programs.
6. Perform other duties as requested by Program Director and Administrative Assistant.
Outreach Coordinator position requirements:
- High-school diploma or GED certificate.
- Experience with the frail elderly and/or developmentally disabled, either as a paid employee or as a volunteer for at least one year. (This may include experience with family or friends.)
- Current CPR card (Adult, Child &Infant) and First Aid Certification
- Ability to work as a member of a team
- TB & Physical clearance to work (within one year prior to hire or within seven days of hire)
- Able to write and speak English and maintain good interpersonal relationships with other members of the team.
- Able to bend, stoop, reach, lift up to 50 lbs. effectively to adequately perform job duties and responsibilities.
- Preferred bilingual (Spanish, Tagalog)
