Summary The position is located in the Department of Veterans Affairs, Veterans Health Administration, Mid-Atlantic Health Care Network (VISN 6), Charles George VA Medical Center, Asheville, NC (CGVAMC).
The CGVAMC is a 1c complexity facility. The incumbent serves as the Program Support Assistant for the Home Based Primary Care (HBPC) and Non-Institutional Care (NIC) programs which are aligned under Primary Care Service.
The incumbent collects and validates information necessary for the appropriate PACT team assignment and enters the necessary data into a variety of databases for use at program, local and national levels.
The incumbent must have sufficient knowledge of the HBPC program and VAMC policy to ensure patients receive continuity of care upon discharge.
The incumbent ensures all processes are accurate, timely, and complete and initiates immediate follow up in response to any incomplete information or inconsistencies among documents.
The incumbent is responsible for the development and maintenance of a database which allows for compiling and tracking of data points necessary for the preparation of reports and the completion of numerous other functions, such as GIS mapping, identifying high priority patients who might need specific assistance in the event of a weather or human-caused emergency, identifying patients on oxygen or ventilation support, and listing of patients who are of Do Not Resuscitate (DNR) status.
The incumbent serves as the PCMM coordinator for patients enrolled in the HBPC program, ensuring adherence to PACT and PCMM policies, guidelines and business rules.
The incumbent ensures that all evaluation procedures are implemented in accordance with current facility, VISN 6, VHA, Joint Commission and other applicable regulatory body rules, regulations and policies.
The incumbent collects, validates and analyzes data from service level -reports, nationally maintained statistics, and supervisory / peer reviews.
The incumbent is responsible for validation of all data to ensure accuracy and obviate delays caused by insufficient or inaccurate responses.
The incumbent's technical and computer skills are used to create, maintain and modify data bases for program support and analysis.
The incumbent has primary responsibility to collect, validate and track all needed data for HBPC and for the numerous programs which are coordinated out of Non-Institutional Care (NIC).
The incumbent has primary responsibility for maintaining the HBPC clinical staff scorecard, which includes monthly data for the various disciplines to include data from national and service level sources.
The incumbent has responsibility to maintain the HBPC vehicle fleet, serving as the liaison between the Facility Fleet Manager, the program leadership, and program staff.
The incumbent serves as a primary timekeeper for the NIC and HBPC staff and is cross trained to provide backup services for Geriatrics & Extended as needed.

Qualifications To qualify for this position, applicants must meet all requirements by the closing date of this announcement.
Time-In-Grade Requirement : Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements by the closing date of this announcement.
For a GS 7 position you must have served 52 weeks at the GS 6. The grade may have been in any occupation, but must have been held in the Federal service.
An SF-50 that shows your time-in-grade eligibility must be submitted with your application materials. If the most recent SF-50 has an effective date within the past year, it may not clearly demonstrate you possess one-year time-in-grade, as required by the announcement.
In this instance, you must provide an additional SF-50 that clearly demonstrates one-year time-in-grade. Note : Time-In-Grade requirements also apply to former Federal employees applying for reinstatement as well as current employees applying for Veterans Employment Opportunities Act of 1998 (VEOA) appointment.
You may qualify based on your experience as described below : Specialized Experience : You must have one year of specialized experience equivalent to at least the next lower grade GS 06 in the normal line of progression for the occupation in the organization.
Examples of specialized experience would typically include, but are not limited to : experience with clerical and administrative procedures, various office skills;
and carrying out day-today administrative support activities; Working in an administrative position which required preparing correspondence, reports and graphs utilizing a variety of computer program.
Use own initiative to follow-up on action items and time-sensitive issues with appropriate staff members. Note : A full year of work is considered to be 35-40 hours of work per week.
Part-time experience will be credited on the basis of time actually spent in appropriate activities. Applicants wishing to receive credit for such experience must indicate clearly the nature of their duties and responsibilities in each position and the number of hours a week spent in such employment.
Physical Requirements : The work is mostly sedentary but requires standing during presentations or training; may require some walking, bending, stretching and carrying objects such as files / binders.
Work also requires spending time performing keyboarding duties. No special physical characteristics are needed to perform the work.
Education There is no educational substitution at this grade level.
