HBPC PACT Case Manager: Reporting directly to the Nurse Manager/Program Director, the HBPC Care Manager is accountable for adhering to established policies, standards of care and practice. They serve as facility champion and coordinates local HBPC PACT implementation, training, communication, outreach and sustainability. The case manager plans, organizes, directs and evaluates the patient on the HBPC PACT team and reports related actions/activities to Program Director and Medical Director

Qualifications:

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.

Basic Requirements:
  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English.
  • Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing.
  • Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
Preferred Experience: Demonstrated possession of advanced practice skills and judgment in providing ongoing patient safety.
Leadership characterized by substantial and continuous responsibility and accountability for population groups or integrated programs that cross service and/or discipline lines that influences the organization mission and health care.

Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:


The full performance level of this vacancy is VN-03. The actual grade at which an applicant may be selected for this vacancy is in the range of VN-01 to VN-03.

Registered Nurses:
  • Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 years of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 years of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.
  • Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years of nursing practice/experience; OR a Doctoral degree and approximately 2-3 years of nursing practice/experience.

Physical Requirements: This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, driving and pushing. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations, operative/invasive procedures, or home settings. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. Must complete annual Employee Health requirements, such as annual TB screening or testing, as a condition of employment. The incumbent who provides care in home settings or other off-site locations may be required to drive and/or ride in GSA-vehicles.

The Nurse Case Manager has on-going responsibility for all the following functions:
  • Triages patient phone calls, requesting health care services at the home care settings.
  • Evaluates and assesses patient's health care needs based on patient's signs and symptoms utilizing established triage protocols and guidelines.
  • Obtains appropriate input and direction from the provider as needed to determine best clinical course of action and recommendations for specific patient health care needs.
  • Accurately documents symptoms/complaints, nursing assessment, advice provided and patient/caller response. Follows policies, SOPs, procedures, and protocols to ensure consistency and departmental effectiveness as well as improve health care outcomes of patients/callers and their access to appropriate health care.
  • Coordinate and schedule same-day home appointments on an as needed basis.
  • Utilize standing orders, triage protocols and algorithms appropriately.
  • Perform other nursing functions as assigned. Assist providers with special procedures as needed.
  • Manage prescription refills per protocol.
  • Participates in Performance Improvement activities as assigned.
  • Assures that pertinent medical information is forwarded to proper departments as needed for continuity of patient care. Obtains all medical documentation from hospitals, home care agencies to ensure continuity of care and follow up as necessary.
  • Patient care duties include but are not limited to: direct nursing care, assessments including disease state and responses to plan of care, lab results, teaching patients and healthcare providers, performing advanced evaluations, and guiding and assessing impact of new workflows/procedures.
  • Works collaboratively to assist with Joint Commission survey readiness and Patient Safety Goals are adhered to at all times.
  • Successfully performs activities appropriate to their grade as outlined in the Nurse Qualifications Standards. Standards identify the educational, experiential and competency requirements for the Registered Nurse at each grade. The Educator will actively maintain specific competencies, seeks specialized competency or other training that enhances his/her abilities, and seeks to apply evidence-based practice that will improve patient outcomes.
Preferred: 3-5 years MED SURG experience or emergency room experience and 1-5 years Primary Care experience, home care experience

Pay: Competitive salary, annual performance bonus, regular salary increases.
Paid Time Off: Receive 13 to 26 days paid time off per year, not to include 10 paid federal holidays; and 13 sick days annual with no limitation on accumulation.
Retirement: Traditional federal pension (5 years' vesting) and federal 401K with up to 5% in contributions by VA.
