What are the responsibilities and job description for the GPD Residential Case Manager position at Veterans Inc.?
With the belief that they were there when we needed them, so we must be there now that they need us, Veterans Inc., the largest provider of supportive services to Veterans and their families in New England, is seeking to fill the Residential Case Manager position and become a valued member of Veterans Inc. team!
This position would be directly responsible for a caseload of homeless male veterans residing in our Transition Care Facility (Grant and Per Diem, GPD). This assistance would be provided through intervention, emergency shelter, housing, employment & training, and supportive services. Must be self-motivated but also able to work within a team and easily take direction.
WHAT YOU'LL BE RESPONSIBLE FOR DAY TO DAY:
Client Assessment: Conducts individual intakes, evaluations, and biopsychosocial assessments of all assigned clients including employment-focused assessment of the individual's skill-set and potential barriers to employment. Evaluate length of stay, maximum of 2 years.
Case Management: Assists with any medical appointments, legal issues/appointments, DTA, and SNAP application and transportation needs/barriers. Develop a supportive relationship with veterans and families, as needed, to assist clients in attaining the highest level of independent functionality. Provides assistance and support through intervention, housing, employment & training, and supportive service outreach, assessment, individualized client care, and service coordination. Encourages housing placement readiness.
Service Coordination: Meets regularly and as scheduled with assigned clients to provide and receive feedback on their progress, ensure quality/improvement of program delivery, and ensure individual professional development towards program/personal goals.
Documentation: Maintains ongoing data collection and reporting as required by Department of Veteran Services, the Department of Labor, and Veterans Administration. Communicates and updates staff and on client progress and resident-related issues. Maintains audit-ready client files and records as required.
Outreach: Establish and maintain resource and referral network to include linkages with housing and resource service providers, homeless service providers, employment service providers, veterans' service providers and organizations, the US Department of Veterans Affairs, and others as appropriate. Provide program awareness information and orientation activities to members of the referral network.
WHAT YOU MUST HAVE:
- Bachelor's degree in Human Services, Social Work or other related field required. Military experience may be considered in lieu of a degree.
- Minimum of two years' experience in Human Services, Vocational Rehabilitation, and Counseling, Social Work or relevant area required.
- Experience working in a human services setting or with veteran population and/or homeless population
- Must be able to pass a CORI background records check.
- Compliance with Veterans Inc. COVID-19 Vaccination Policy. Religious and Medical Exemptions are considered.
