Position Responsibilities:
Service Activities:
- Conduct assessment interviews with veteran clients
- Assist veterans with the creation and implementation of Individualized Service Plans (ISP) which incorporate housing, employment, educational and income/financial goals
- Track Veterans' progress through PA-WITS system and modify ISPs regularly
- Assist Veterans in finding a path of recovery that best fits their individual needs and assure program compliance
- Assist with coordination of resources within the continuum of care
Clinical Activities:
- Perform case finding functions and outreach, identifying and screening patients for Psycho-social needs.
- Conduct assessments and develop treatment plans in collaboration with the Veteran Administration and other provider organizations.
- Coordinate referrals of Veterans to community-based Providers for additional services based on needs and eligibility (this includes expanding the existing referral network as necessary).
- Maintain case records in accordance with program guidelines and professional requirements.
- Implement legal and ethical standards of service delivery, privacy, and confidentiality, complying with all applicable regulations
Program Development:
- Facilitate housing search with Veterans clients and coordinate supportive services with applicable providers (if applicable)
- Ensure that Veterans Place employment and training services are synchronized with other community activities.
- Oversee the delivery of services within the scope of this program and make recommendations to Program Manager
- Work closely with the Veterans Place staff to ensure a high quality of services.
Communication:
- Prepare reports as requested by other Veterans Place staff.
- Document and track Veteran progress in PA-WITS system.
- Participate in administrative, staff, clinical, and in-service activities and training as required.
- Communicate policies and the rationale behind them to residents.
- Inform other agency staff members of relevant matters related to the program.

Qualifications:
- Bachelor's degree in related field or equivalent in experience
- Knowledge of Substance Abuse Disorder and resources for treatment, including harm reduction
- Experience working directly with the homeless population strongly preferred
- Experience working with Veterans Administration preferred
- Must have a valid driver's license
- Veteran Status preferred
- Passing a pre-employment drug and alcohol screening is required and a background check will be conducted
