What are the responsibilities and job description for the Care Manager position at VHS Recruitment?
Suburban Geriatrics is the premier provider for Geriatric care in the Northeast. We specializing in treating residents in long term care and assisted living communities. We are currently seeking social worker / case manager to work with our residents in facilities to coordinate all aspects of care, work closely with our intake coordinator and clinical staff to facilitate required paperwork, insurances, DME's, schedules and non-clinical needs. This candidate should appreciate working with the elderly population, be well-organized and feel comfortable making phone calls and using EMR. Must be willing to travel.
Previous experience in case management or a healthcare setting highly preferred.
A BSW a plus, but not required.
