Responsibilities may include:
- Provide patient visits as scheduled and complete patient care within the designated timeframes.
- Complete all documentation in an accurate and complete manner.
- Provide, modify, or discontinue medical social work modalities and interventions based on ongoing assessment/reassessment of the patient’s clinical status and in accordance with the plan of care.
- Complete psychosocial evaluations of patients as assigned.
- Collaborate with all members of the interdisciplinary team to ensure the patients’ psychosocial needs are addressed on an ongoing basis.
- Assist with goal setting based on care discussions with patient and family.
- Assist in the execution of the care plan.
Requirements for this position include:
- Masters degree in Social Work required.
- License in Social Work from State of Pennsylvania preferred.
- Experience in Medical Social Work is preferred.
- Experience within the Hospice field is preferred.
- Valid driver's license, car insurance and reliable transportation.
- Willingness to travel throughout assigned service area.
