The key function of this role is to assist referral agencies with patient placement through an efficient, effective and focused review of a patient’s clinical needs and financial status. Throughout this review, the Villa Referral Coordinator will ensure that all pre-admission paperwork and verifications have been completed. This person is responsible for ensuring a smooth transition for the patient from hospital to the Villa Center through excellent communication with facility staff and referring agencies.

ESSENTIAL FUNCTIONS
Functional:
Under the supervision of the Villa Referral Manager, assumes responsibility for review of referrals and coordination of clinically and financially appropriate admissions
Works with the referral source to obtain all necessary clinical information and with Villa Clinical Manager to assess clinical appropriateness of all referrals.
Coordinates with the Director of Business Development, Villa Clinical Manager or Clinical Liaison for on site assessment when deemed necessary.
Completes all necessary financial verifications in order to verify all existing health insurance benefits and confirm the primary and secondary payer sources.
Understands the Managed Care policy and authorization requirements. Works with Managed Care Coordinator to process admissions with any payers requiring prior authorization.
Runs criminal and sex offender back ground checks to identify any potential offenders.
Demonstrates efficiency and effectiveness while reviewing referrals.
Communicates with facility staff and assigned physicians regarding patient admissions and ensures all relevant information has been shared including: diagnosis, special needs, medical equipment, etc.
Ensures that all referrals/inquiries for admission and readmission receive a response within 15 minutes. This includes referrals through all avenues including, but not limited to: eFax, Ecin, phone, in person, etc.
Communicates with referral sources and/or Business Development Team members in a timely manner on the acceptance or denial of all referrals.
Creates profiles for new admissions and updates existing profiles upon re-admissions within Villa’s electronic health record system (Point Click Care).
Completes the Referral Tracking Log on a daily basis to reflect all referral activity and updates.
Ensures effective, efficient and professional communication occurs with all interactions.
Follow established communication expectations with team members, facility staff, and organizational leadership and referral sources.
MINIMUM QUALIFICATIONS
Education: High School diploma or equivalent.
Experience:
At least 2 to 3 years prior SNF Admissions experience.
High level of PC literacy preferred (Microsoft Office version 1997, 2000 or XP, MS Word, MS Access, MS Excel, E-mail, Outlook, Internet Explorer)
Ability to read, write, speak and understand English.
