What are the responsibilities and job description for the Social Worker position at Vincentian?
Summary:
The Social Worker is responsible for assisting with all Social Service functions including preparation of resident social histories, identification of psychosocial needs and maintenance of progress notes. Also acts as liaison between resident family member or caregiver, and facility staff. Promotes the preservation of the resident's mental, emotional and social well being.

Essential Duties and Responsibilities:
1. Commits to the Vincentian Collaborative System (VCS) mission to nurture and sustain a ministry of compassionate care that preserves the human dignity of persons within a diverse and changing society.
2. Upholds and promotes the VCS values of spirituality, compassion, quality, dignity, collaboration, advocacy and stewardship.
3. Promotes and supports a resident centered care environment that ensures the treatment of all residents, family members, visitors, fellow employees and customers with kindness, respect, and dignity.
4. Adheres to all facility and department work rules, policies and procedures.
5. Participates in interdisciplinary care plan meetings for assigned residents.
6. Participates in expectation meeting to gather needed information on all new residents.
7. Participates in resident care plan process.
8. Completes Social Service component of MDS, progress notes and makes changes as appropriate.
9. Provides support and assistance to residents and family representatives during their stay, including the initial adjustment period.
10. Communicates and resolves any issues or concerns with the medical staff, nursing and other appropriate staff.
11. Responsible for the referral of residents to social, health, and community agencies. Ensures documentation concerning such referrals.
12. Resident advocate and communication liaison between resident/families and staff/community services.
13. Make arrangements for obtaining necessary adaptive equipment, clothing, personal items, and other services as needed, as required.
14. Counsels in areas of adjustment, grief, loss issues, depression and anxiety. Refers to appropriate professional services as needed.
15. Coordinate discharge planning with other departments, outside services, resident and family.
16. Assist in preparation, notification, and adjustment to room changes; mediating room/roommates issues.
17. Maintain marketing materials and other information on community services.
18. Assist with resident and family orientation.
19. Performs all other duties assigned by management.
20. Required to support the organization's Code of Ethical Conduct and Compliance Program by timely reporting any actual or suspected compliance, fraud and abuse, ethics or policy violations.
Education/Experience:
1. Bachelor of Science in Social Work or related field required.
2. Minimum of one to three years of social work experience in long term care or other health care setting required.
