Job Description
Schedule : Per diem, hours vary
Salary : $ / hour
The Per Diem Professional Program staff provides individual and group support to clients in a residential or day program.
Teach skills to enhance independence and quality of life. Function in a professional role in order to fulfill programmatic needs / routines.
The Per Diem Professional Program staff has a range of duties including :
- Assist in developing, implementing, and documenting individual treatment support plans.
- Responsible for a variety of daily tasks with clients. This includes rehabilitative interventions, behavioral interventions and monitoring, scheduling appointments, monitoring progress, providing transportation, and other related tasks.
- Provide role model and support to clients. Assist them in becoming fully integrated into their communities.
- Assess clients' physical and emotional status, and report changes to supervisor
- Ensure program site safety and maintenance as necessary at all times. Oversee and participate in the daily living and cleaning of the program, inside and outside with clients.
- Perform other related duties as required.

Requirements
- High school diploma or equivalent is required
- CPR and First Aid may be required within two weeks of hire
- MAP may be required with 90 days of hire
- Safety Care may be required within 90 days of hire
- Other training as assigned
- Must be able to successfully pass a multistate CORI, SORI, reference check, employment verification, education and credentialing background check
Driving Requirements
Drivers License check required; vehicle access preferred
