What are the responsibilities and job description for the Case Manager position at Visiting Angels?
Job description as follows but not limited to:
- Follow up on lead calls and conduct telephone and face to face interviews with prospective clients.
- Effectively resolve caregivers and clients concerns.
- Assess caregiver performance ,participate in caregiver performance reviews.
- Perform general office duties such as telephone services, filing and scheduling.
- Develop the care plan in accordance with client's needs.
- Maintain positive relationships with clients , caregivers and referral sources.
Qualifications
- knowledge in a health care related field and at least one year of supervisory experience.
- Previous experience in caregiving or case management
- Proficiency with Microsoft Office application and scheduling software such as Clearcare.
- Strong supervisory skills, including the ability to provide guidance to caregivers
- Must have valid driver's license and reliable transportation.
- Must be able to maintain verbal and written communication with co workers , clients, family members and leadership team.
- Ability to present a well groomed image that reflects the professionalism of the business.
- Ability to work independently and as part of a team
