What are the responsibilities and job description for the In-Home Social Worker ( LCSW) position at Visiting Counselors of New York, LCSW, PLLC?
We are seeking a highly motivated and experienced Licensed Clinical Social Worker (LCSW) to provide in-home counseling services to older adults in Nassau County. The LCSW will be responsible for conducting assessments, developing treatment plans, and providing individual and/or family therapy to clients in their homes.
Key Responsibilities:
- Conduct comprehensive assessments of clients' social, emotional, and psychological needs
- Develop individualized treatment plans and set goals in collaboration with clients and their families
- Provide individual and/or family therapy to clients in their homes
- Monitor clients' progress and make adjustments to treatment plans as needed
- Communicate with other healthcare providers to ensure continuity of care
Qualifications:
- Must hold a valid LCSW license in the state of New York
- Must have experience working with older adults
- Strong clinical and diagnostic skills
- Excellent communication and interpersonal skills
- Must be able to work independently and as part of a team
