Essential Job Duties
- Interacts with co-workers in an appropriate manner to facilitate teamwork.
- Oversees scheduling of nursing team members.
A. Manages overtime and productivity.
- Assists in marketing agency programs to area hospitals, physicians, nursing homes, rehabilitation facilities, and other referral sources.
- Monitors nursing overtime hours and takes action to alleviate overtime hours.
- Works cooperatively with other supervisors to develop weekend and on-call schedules for staff.
- Assures that team members turn in all documentation timely and accurately.
- Performs necessary reviews of documentation as established by agency policy and communicates findings appropriately.
- Establishes monitoring to assure that quality and safe home care services are provided and maintained by reviewing clinical records, conducting supervisory home visits of team member, and conducting quality/satisfaction calls routinely to those served.
- Assists with quarterly utilization review per agency policy.
- Facilitates weekly IDG conferences with all team members and documents IDG summaries appropriately.
- Reviews medical records as indicated to assure appropriate frequency and to access goals, service utilization and appropriate treatment plan.
- Actively plays a role in QAPI.
- Assures compliance with State and Federal regulations for patient care at all times.
- Provides direct patient care when appropriate.
- Assumes on call responsibility as assigned.
- Serves as a key communication link between staff and agency administration to assure and promote two-way communication.
- Demonstrates fiscal accountability in making decisions regarding clinical responsibilities.
- Understands and assures the use of policies, procedures, and other measures to maintain efficiency and accuracy in care delivery.
- Recommends and implements changes in agency policy and procedure to facilitate quality care and maintenance of the expected standard of care.
- Performs performance evaluations on those supervised in the mandatory time frame.
- Completes supervisory visits and competency checks as required.
- Assists with providing education to staff and makes plans to meet educational needs of team members.
- Accepts patient referrals consistent with the agency admission policy, evaluating the potential patient for eligibility of care in her/her place of residence as indicated.
- Plans and implements procedures which will promote efficient caseload management by the clinical staff.
- Communicated with doctors, patients and families, health institutions, insurance companies and others to enhance a positive image of the organization.
- Represents the organization in community education on health issues and Hospice services.
- Documents and follows up on patient complaints through appropriate channels.
- Makes informational visits as needed.
- Is knowledgeable regarding agency policies, procedures and general information.
- Attends selected workshops and seminars to enhance job performance.
- Assists in covering clinical calls due to lunch hour (rotation), breaks, and other times as needed.
- Reports to work at the scheduled times.
- Willingly performs other duties outside of job responsibilities as may be required.
Education, Training & Experience
- Bachelor’s degree in nursing preferred; prior supervisory experience in health care industry required; home care or hospice experience preferred.
- Highly developed organizational, planning, writing skills and oral communication skills.
- Understands the significance and importance of setting and meeting goals.

- Able to set a realistic schedule and complete deadlines.
- Flexible in work schedule as required.
