JOB DESCRIPTION

Provide service to homebound patients who require occupational therapy to regain physical and/or mental functioning and/or adjustment to their handicaps, per order of attending physician. Provide occupational therapy services to hospice patients as ordered in the patient’s plan of care at the request of the Therapy Manager.


ESSENTIAL FUNCTIONS AND RESPONSIBILITIES

  • Evaluate patient’s functional status (muscle function, endurance, visual coordination, written and verbal communication skills, self-care ability, work capacity, etc.)
  • Evaluate home environment for hazards or barriers to more independent living.
  • Identify equipment needs.
  • Participate in the development of the total plan of care.
  • For patient who plans to return to work, the occupational therapist may perform work capacity evaluation and may refer to specialized vocational training in accordance with agency policy.
  • Develop treatment program and establish goals for improved function. Communicates plan of care to referring physician and other agency professionals.
  • May teach new skills or retrain patient in once familiar daily activities that have been lost due to illness or injury, in accordance with agency policy.
  • Develop needed plans for modifying equipment, appliances and physical surroundings in the home.
  • May train patient in the use of prosthetic and/or orthotic devices.
  • Maintain appropriate clinical records, clinical notes, and reports to physician any changes in the patient’s condition. Submits these records in accordance with agency policy.
  • Maintain contact/coordination with other personnel involved in the patient’s care to promote coordinated, efficient care.
  • Attend and contribute to in-services, case conferences, and other meetings as required by agency policy to ensure coordinated and comprehensive plans of care for the patients of the agency.
  • Identify patient and family/caregiver needs for other home health services and refers, as appropriate.
  • Instruct patient’s family/caregiver and other agency health care personnel in patient’s treatment regime when indicated and appropriate.
  • Supervise certified occupational therapy assistants according to agency policy and state regulations.
  • Instruct, supervise and evaluate home health aide care every two (2) weeks when therapy is the only skilled service.
  • Accepts personal responsibility for annual CE in the practice of Occupational Therapy.
  • Perform other duties assigned.

Requirements

EDUCATION AND EXPERIENCE

  • Master’s degree from an accredited school of Occupational Therapy.
  • Current license to practice as an Occupational Therapist in the State of New Hampshire is required.
  • Minimum of 2 years clinical experience as an Occupational Therapist.
  • Meets the standards and the credentialing requirements as outlined in the Conditions of Participation 484.115.
  • Registered with the National Registration Examination of the American Occupational Therapy Association.

SKILLS AND ABILITIES

  • Ability to deliver occupational therapy in a manner that is appropriate to patient’s age, physical ability and intellectual development, as defined by the plan of care.
  • Must demonstrate proficiency in assessing treatment and responses and adapting to meet the needs of the client population served.
  • Ability to verbally communicate to express or exchange ideas and to convey detailed or important spoken instructions to patients, physicians, families and other employees.
  • Ability to participate in the development of an occupational therapy plan of care.
  • Ability to perform job independently, when required.
  • Ability to demonstrate courtesy, compassion and respect in relating to patients, families, visitors, physicians and co-workers.
  • Ability to work as a team and maintain positive working relationships with employees and healthcare professionals.
  • Ability to document patient care accurately according to healthcare standards and plan of care.
  • Working knowledge of Microsoft Office software and other industry specific programs.
  • Must demonstrate ethical conduct and build trust with patients and family members.
  • Ability to follow safety guidelines as instructed by the agency.
  • Ability to wear protective clothing such as mask, glove, head cover, safety glasses, etc.
  • Ability to travel required. Travel throughout central New Hampshire is required.
  • Current drivers’ license is required.
  • Proof of automobile insurance is required.
  • Ability to pass a motor vehicle background screening is required.

PHYSICAL DEMANDS/WORK ENVIRONMENT

  • Job requires physical activities including but not limited to; standing for extended periods of time, bending, lifting, reaching, kneeling, pushing, pulling, crouching, crawling, fingering, grasping, feeling, talking and hearing.
  • Job requires moderate physical work including the ability to exert up to 50 lbs. of force occasionally, 30 lbs. regularly.
  • Job requires close visual acuity including the ability to see at or within arm’s reach.
  • Job is subject to inside environmental conditions; low noise level.
