Patient Enrollment Specialist, Fulltime, Dayshift, SW Fort Wayne

POSITION OBJECTIVE:
In this role, the Patient Enrollment Specialist will receive and process new client referrals being called in by facility discharge planners, social workers, medical professionals, and community members. The intake representative uses their top-notch customer service and telephone skills to help ensure that referrals are handled in a timely and caring manner so that our patients can receive the best possible care.

QUALIFICATIONS:

• Two years experience in customer service.
• Clerical experience with a basic understanding of Microsoft Office.
• Previous experience in health care preferred.
• Ability to prioritize and multitask.
• Excellent verbal and written communication skills
• Must exhibit excellent interpersonal and public relations skills to effectively interact with patients and family members, physicians, and other members of the healthcare community.
• Must be a self-starter with a high level of accountability and responsibility.
• Must be able to work independently and within a team environment, be flexible, highly organized, and able to prioritize their work.
• Must be able to plan and implement changes and flex between tasks as necessary.
• Must be able to safely perform the essential functions of the job without posing a direct threat to the health and/or safety of self or others in the workplace.
• High school diploma or GED equivalent.
• Must have an Indiana motor vehicle operator’s license and have access to an automobile with current automobile insurance coverage.
• Must have basic computer skills.

ESSENTIAL FUNCTIONS:

• Answers telephone lines promptly and efficiently and respond to message content appropriately.
• Listens compassionately and communicates clearly and concisely all Stillwater programs offered and provides basic education on Stillwater-offered services.
• Receives referrals from any referral source, including physicians, hospitals, nursing homes, assisted living, independent living, and the community in general, and initiates the intake process.
• Establishes and maintains positive working relationships with current and potential referral sources.
• Maintains professional, effective internal and external working relationships that meet agency values.
• Demonstrates positive, informative interaction with co-workers, referral sources, and patients/families.
• Enters new patient records in the system, ensuring the accuracy of all patient demographic information.
• Reports pertinent data regarding referrals to admission nurses. 
• Adheres to Stillwater privacy practices as described in the Notice of Privacy Practices.
• Performs other similar or related duties as requested or assigned by the supervisor.
