What are the responsibilities and job description for the Day Program Supervisor position at Vista Care?
AREA OF RESPONSIBILITY:
Under the direction of the Area Director or Program Service Coordinator, the Day Program Service and Support Director (SSD) is responsible for the daily operations of Vista Care's Day Program, to ensure the quality of programming, safety and wellbeing of all individuals served.
DUTIES:
- Monitor the overall quality of assigned program. Ensure timely corrective actions when quality problems are identified.
- Responsible for assisting in and/or the planning and coordination of daily personal cares, personal hygiene, medical needs, diabetic and nutritional requirements, activities, and community outings. Also, individual support and transportation requirements of the participants at all times while they are under the care and supervision of Pathways.
- Responsible to assist with or supervise activities of daily living to include but not limited to the following: toileting, catheterization, glucose monitoring, injections, oral care, range of motion, transfers, food preparation, medication administration, personal shaving.
- Assist in or develop activities and techniques that can increase the quality of life for all individuals attending.
- Required to report all incidents involving aggression, behavioral intervention, medical situations, or treatments relating to staff or individuals, any pertinent information to the Program Service Coordinator or Area Director in a timely manner.
- Conduct staffing meetings.
- Develop ISP plans and programs for individuals served
- Update and oversee all information on each individual to keep it up to date.
- Is responsible for communicating with all members of the team in regards to changes of condition, incidents etc. for our individuals and ensure the appropriate documentation is completed in a timely manner.
- Work and maintain a flexible schedule, provide direct care and emergency coverage when required for both staffing and routes
- Initiate and maintain contact with the person served, family and support team members.Schedule maintenance and repairs for company vehicles used to transport individuals in their assigned county.
- Oversight of money management for persons served as required by Vista Care policies, proecedures, ISP and applicable state and federal laws.
- Provide direct supervision and oversight to staff, including monitoring of performance, provision of feedback, evaluation, disciplinary action, timecard approval, etc.
- Assure that all staff are working within the company policies and are trained to care for all persons served according to the ISP and within their medical, physical, psychological and social needs.
- Monitor the physical site maintenance and improvement, assuring compliance with regulatory agencies, person served safety and organizational standards, taking immediate action to communicate maintenance issues and correct problems, as they occur.
