Job Overview: We are seeking an experienced healthcare professional to fill the position of Chronic Care Manager Supervisor. Responsibilities:
Supervision and Leadership:
- Provide effective leadership and guidance to a team of chronic care managers with diverse experience in different healthcare facilities.
- Conduct regular performance evaluations and provide constructive feedback.
- Foster a collaborative and supportive team environment.
Chronic Care Management Across Settings:
- Utilize your experience to adapt and implement chronic care management strategies for various healthcare facilities, including hospitals, clinics, and long-term care facilities.
- Ensure care plans are tailored to meet the specific needs of patients in different settings.
- Collaborate with facility staff to optimize care coordination and patient outcomes.
Quality Assurance:
- Monitor and evaluate the quality of care provided by the chronic care management team across different healthcare facilities.

- Ensure compliance with healthcare regulations and standards applicable to diverse care environments.
Team Training and Development:
- Conduct ongoing training sessions to address the nuances of chronic care management in various healthcare facilities.

Collaboration:
- Collaborate closely with facility staff, including nurses, administrators, and support personnel, to ensure seamless and effective care delivery.

Data Management:

Qualifications:
- Bachelor's degree in a healthcare-related field; Master's degree preferred.
- Prior experience of chronic care managment a must.
- Minimum of 3 years of experience in chronic care management.
- At least 1 year of experience in a supervisory or managerial role.
- Strong understanding of chronic health conditions and evidence-based interventions in different care settings.
- Excellent communication, leadership, and interpersonal skills.
