What are the responsibilities and job description for the Nurse Practitioner (NP) - Hospice position at VitalCaring Group?
The Nurse Practitioner is a registered and licensed professional with an advanced graduate education who is in compliance with the state Nurse Practice Act and under the supervision of a collaborating physician. They provide hospice care through the evaluation and assessment of the overall medical conditions of patients. In collaboration with the Hospice Medical Director, a licensed physician, and the Inter Disciplinary Group they visit patients, complete face to face assessments and evaluate hospice eligibility as assigned by the hospice. The nurse practitioner also uses any community documentation and collaboration to assist in coordinating care and establishing hospice eligibility.
Responsibilities
- Provides medical care, palliates, and manages terminal illness in compliance with the state Nurse Practice Act under the supervision of a collaborating physician.
- Completes History and Physical examinations by evaluating the patient’s medical condition and health history, ordering hand interpreting results from laboratory and diagnostic tests or procedures, diagnosing health conditions and document the findings in the patient’s medical record.
- Manages health conditions and terminal illnesses by developing an individualized plan of care, prescribing medication and/or treatments, obtaining consultations or making referral and coordinating hospice care services.
- Completes the hospice face-to-face (F2F) visit requirements prior to the third and subsequent hospice benefit periods to assess hospice eligibility based on the hospice conditions of participation and the local coverage determinations. Provides clinical information to the hospice medical director for interpretation for eligibility for hospice recertification.
- Makes recommendations to the hospice medical director regarding patient eligibility for certification and recertification for hospice care.
- Reviews clinical information, coordinates and oversees patient care in consultation with hospice interdisciplinary team and other healthcare practitioners.
- Assures that patient receives appropriate measures to control symptoms, through collaboration with interdisciplinary team members.
- Reviews and develops protocols for treatment and proposes options for interventions based on the literature in collaboration with the collaborating physician.
- Consults the collaborating physician when the patient’s plan of care is outside standardized practice and protocols.
- Communicates and collaborates with the interdisciplinary group to create, review, and revise the patient’s plan of care.
- Consults with the patient’s attending physician, the hospice medical director and the other healthcare practitioners regarding the patient’s eligibility for hospice care, ongoing care needs and medical management related to the patient’s terminal condition.
- Communicates with other community health practitioners to coordinate the plan of care.
- Attends and participates in hospice interdisciplinary group meetings.
- Ensures compliance with the Medicare conditions of participation and other state regulations govern the provision of healthcare.
- Participates in organizational monitoring of the quality of medical services and quality improvement initiatives.
- Meets mandatory continuing education requirements of the Agency and licensing board.
- Demonstrate knowledge in communication and counseling patient/family in dealing with end-of-life issues.
- Uses effective interpersonal relations and communication skills; facilitates the use of these skills by other team members to achieve desirable outcomes.
- Participates in Agency-sponsored in-service training.
Physical, Mental, Miscellaneous Demands
- Prolonged sitting required, with the ability to operate office equipment which may include heavy lifting, bending, and standing.
- Requires the ability to handle stressful situations in a calm and courteous manner at all times. If required to make patient visits, prolonged standing and walking required, with the ability to lift up to 50 lbs. and move patients.
- Requires working under some stressful conditions to meet deadlines and Agency needs.
- Must have and maintain valid Driver’s License, and proof of current valid auto liability insurance.
- Travel to various geographic locations required. Some overnight stays required.
- Requires current CPR certificate, and the ability to maintain a current CPR certificate.
- Requires ability to use up to 50 pounds of force occasionally and/or up to 20 pounds of force frequently and/or up to 10 pounds continuously to move objects.
- Work deals mostly with areas such as preparing and reading data and figures, records, reports, visual inspection involving small details. Although important, depth perception and field of vision (peripheral) are not as critical as the ability to distinguish small details and markings very near to the observer.
- Requires ability to grasp, pull, push, carry or otherwise manipulate objects; ability to perform tasks requiring action of muscles or groups of muscles and foot and/or hand-eye coordination.
- Requires ability to communicate clearly and make self-understood effectively in face-to-face interactions; articulate with accuracy to communicate using phone.
- Requires ability to hear and receive verbal instructions correctly, answer phones, communicate professionally with people in situations with some background noise.
- Requires ability to concentrate on fine detail with frequent interruption; ability to focus attention on tasks for 10-20 minutes at a time on a continuous basis, 20-60 minutes on occasion.
- Requires ability to understand and relate to specific ideas one at a time and to the concepts behind specific ideas; ability to remember verbal and written tasks/assignments from a few hours to several day periods.
- Position requires sitting approximately 60% of time standing/stooping/bending/ climbing approximately 20% of time; and walking approximately 20% of time.
- Excellent computer software skills necessary to produce accurate documents and materials required.
- Working knowledge of MS Office, Excel or similar software required. Proficient knowledge preferred.
- Requires ability to work effectively under minimal supervision; exercise excellent, sound professional judgment and maintain confidentiality.
Qualifications
Education/Licensure: Required to be a licensed Nurse Practitioner with the State Nurse Practice Act Licensed in the state of operations where assigned branch is located.
Experience: Minimum of one year of Nurse Practitioner experience required. Prefer a minimum of six months to one year of experience in hospice or pain and symptom management
Skills: Nursing skills as defined as generally accepted standards of practice. Excellent interpersonal skills and the ability to communicate effectively with all segments of the staff. Demonstrates proven decision-making skills. Proof of current CPR.
Transportation: Reliable transportation. Valid and current auto liability insurance.
Environmental/ Working Conditions: Performs duties in an office environment during Agency operating hours. Will be required to make patient visits, may work in patients’ homes in various conditions; possible exposure to blood and bodily fluids and infectious diseases; the ability to work a flexible schedule; the ability to travel locally; some exposure to unpleasant weather; PRN emergency call. Works in an office environment, promoting efficient functioning and coordination of all Agency activities to ensure the highest level of professional patient care.
