TITLE: Licensed Practical/Vocational Nurse
REPORTS TO: Administrator and/or Clinical Director
FLSA: Non-Exempt
JOB SUMMARY
Provide skilled professional nursing care to patient/family/caregiver as directed by the supervising Case Manager or the Patient Care Manager and in compliance with all applicable laws and regulations and with Vitality’s policies and procedures. Provide timely documentation of patient services based on plan of care. Identify other physical, psychological, social and spiritual needs for assigned patients. Work closely with other members of the interdisciplinary team to ensure all patient and family care needs are being met. Adhere to Company policies and procedures and regulatory compliance requirements and directives. Manage related expenditures in a fiscally responsible manner in accordance with the company’s budget. Travel as necessary.
QUALIFICATIONS
1. Minimum age requirement of 18.
2. LPN licensed in the state where program is located.
3. Graduate of accredited practical nurse or vocational nursing program.
4. Minimum of one-year general nursing experience preferred. Community health/Hospice or medical/surgical experience preferred.
5. Compliance with accepted professional standards and practices.
6. Understanding of Hospice philosophy and needs of terminally ill.
7. Good physical stamina. Has presented a pre-employment physician’s health certification including a negative TB skin test and/or CXR and other tests as required by Vitality’s policy and/or state law.
8. Physical exam in accordance with state law requirements.
9. Valid driver’s license with maintenance of good driving record and reliable transportation that is insured in accordance with state and/or Vitality requirements.
10. Satisfactory references from employers and/or professional peers.
11. Ability to work in an interdisciplinary setting.
12. Satisfactory criminal background check.
13. Self-directed with ability to work with little supervision.
14. Demonstrate excellent observation, good nursing judgment and communication skills. Ability to provide good written documentation in a timely manner.
15. Flexible and cooperative in fulfilling all obligations.
RESPONSIBILITIES
1. Work within the scope of the position, in coordination with management, to meet the needs of our patients, families and professional colleagues
2. Ensure quality and safe delivery of home Hospice services to patients/families/caregivers.
3. Implement nursing plan of care as directed by the interdisciplinary team and Patient Care Manage4. Provide professional nursing care by utilizing all elements of nursing process as directed by the plan of care.
4. Provide accurate and timely documentation of patient services to reflect the plan of care.
5. Responsible for checking and ordering medications.
6. Assess and provide patient/family/caregiver education and information pertinent to diagnosis and self-plan of care.
7. Participate in coordination of Hospice services, appropriately communicating and reporting the need for Hospice interdisciplinary team members (e.g., OT, PT, MSW, ST, Dietitian) to the Case Manager or Patient Care Manager.
8. Order and use equipment and supplies effectively and efficiently.
9.Provide appropriate pain/symptom management. Evaluate patient’s response treatments/medications.
10. Complete and maintain all patient records at patient’s residence and submit accurate and relevant clinical notes regarding patient’s condition and care given.
11. Observe patient for changes in condition and report changes and patient’s/family’s/caregiver’s needs to Case Manager or appropriate supervisory personnel. Record pain/symptom management changes/outcomes as appropriate.
12. Assist patient with activities of daily living (e.g., reposition or move patients, bathing, feeding). Facilitate patient’s efforts toward self-sufficiency and optimal comfort care.
13. Teach patient/family/caregiver self-care techniques as appropriate, working in conjunction with other Vitality interdisciplinary team members.
14. Provide and maintain a safe environment for the patient.
15. Fulfill productivity requirements on an average daily census basis as determined by acuity level and patient care management.
16. Assist patient/family/caregiver and other team members in providing continuity of care.
17. Work in cooperation with family/caregiver/and interdisciplinary team members to meet the emotional needs of the patient/family/caregiver.
18. Coordinate travel time for visits and manage a caseload to deliver care in a cost-effective manner that demonstrates an understanding of hospice reimbursement methodologies.
19. Participate in evening/weekend on-call and overtime as required, conducting on-call services in a clinically competent and responsive manner.
20. Participate in staff meetings, department meetings, team meetings, briefings, in-services, committees and other related activities as needed.
21. Maintain clinical records, statistics, reports and records for purposes of evaluation and reporting of agency activities as prescribed and in compliance with local, state and federal laws.
22. Assure for compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures.
23. Maintain all appropriate state nursing licensing requirements. Attend continuing education courses.
24. Regular attendance is to be maintained.
25. Adherence to a code of conduct conducive with Vitality Hospice policy is expected.
26. Meet or exceed delivery of Company Service Standards in a consistent fashion.
27. Interact with all staff in a positive and motivational fashion supporting the Company’s mission.
28. Conduct all business activities in a professional and ethical manner.

WORKING CONDITIONS
General office, extended care facility, patient home setting, exposure to infectious diseases, travel by automobile and occasionally by airplane.
PHYSICAL DEMANDS
Requires physical exertion. Duties of patient care during transfer activities (adjusting the patient in bed, transferring the patient from bed to wheelchair or vice versa, and chair to toilet, etc.) require that the employee have the physical ability to maneuver patients according to the care required. Additional physical abilities required during these activities will vary according to the level of patient cooperativeness, patient weight, available space and available equipment. Regularly required to use hands, talk or hear. Specific vision abilities include close, peripheral, depth perception, and ability to adjust focus. Must be able to travel to businesses, offices, or patient homes or where conditions may be adverse due to weather, environment, or patients’ lifestyles.
