POSITION DESCRIPTIONPOSITION TITLE: Social WorkerREPORTS TO: Social Services DirectorDEPARTMENT: Social Services Department The primary responsibility of your job position is to assist in planning, developing, organizing, implementing, evaluating, and directing our facility’s social service program following the current federal, state and local standards, guidelines and regulations that govern long term care and assisted living facilities. You are entrusted to ensure that the medically related and emotional social needs of the resident are met and maintained on an individual basis.KEY RESPONSIBILITIES:Interview residents, or family members, on admission to obtain social history, and assess memory and depressionFacilitate care conferences with IDT, patient, and family on admission to review and discuss plan of careCoordinate with community agencies, such as home health, palliative care, hospice, and durable medical equipment companies to establish safe discharge plansAssist with planning, organizing, developing, and directing the overall operation of the Social Services DepartmentAssist with developing and implementing policies and procedures for the identification of the medically related and emotional needs of the residentMay assist with Access-A-Ride and Assisted Living applicationsCommunicate the department’s policies and procedures to employees, residents, visitors, government agencies, etcProvide consultation to members of our staff, community agencies, etc., in efforts to solve the needs and problems of the residentMaintain a quality working relationship with the medical profession and other health related facilitiesRecord and maintain regular Social Service progress notes indicating response on treatment plansCompile and maintain a social service directory listingAssist in arranging transportation to other facilities when necessaryAssist with reviewing and developing a plan for social service deficiencies noted during survey inspectionsMake recommendations on changes in policies and procedures to the AdministratorAssist in developing and maintaining a community and social services file of agencies and organizations and organizations that provide assistance to preferred EDUCATION:A Bachelor’s Degree from an approved school of Social Work SKILLS AND KNOWLEDGE:Must demonstrate the knowledge and skills necessary to provide care appropriate to the age-related needs of the residents servedMust be knowledgeable of laws, regulations, and guidelines pertaining to hospital and nursing facility administrative proceduresMust be knowledgeable of computer systems, system applications, and other office equipmentJOB TRAINING:How long it usually takes to completely perform this job when the incumbent has the minimum knowledge, skills, ability, work experience and/or education: up to 3 months PHYSICAL REQUIREMENTS:Must be able to relate to and work with ill, disabled, elderly, emotionally upset, and at times hostile people within the facilityMust meet the general health requirements set forth by the policies of this facility, which include a mental and physical examinationMust possess sight/hearing senses, or use prosthetics that will enable these senses to function adequately so that the requirements of this position can be fully metMust be able to push, pull, move, and /or lift a minimum of 25 pounds to a minimum height of 5 feet and be able to push, pull, move, and/or carry such weight a minimum distance of 50 feet May be necessary to assist in the evacuation of residents during emergency situationsWORK ENVIRONMENT: Works in all areas of
