We are a senior health care, solution driven company servicing 30 facilities in the state of Colorado.                  WHAT’S IN IT FOR YOU AS A VIVAGE EMPLOYEE?Competitive Salary Flexible working hoursDaily Pay Advance Option AvailableHoliday Pay (when working on a major holiday)Sick time pay accrual  Paid Time Off Health, Dental, Vision, and More OVERVIEW OF THE ROLEThe primary responsibility of your job position is to plan, organize, develop, and manage operations relevant to nursing with managed communities, consulting contracts and special projects.  Provides information and makes recommendations related to resident care and facility practices following the current federal, state and local standards, guidelines and regulations that govern long term care and assisted living facilities.   KEY RESPONSIBILITIES:Assures implementation of company programs, systems, standards and policies in all managed communities. Assists with the evaluation of the facility’s nursing department including nursing management and makes recommendations for training, education and corrective action. Assesses the status of resident care delivery and provides approaches for enhancement. Develops and maintains a strong working relationship with company Corporate Medical Director as well as facility Medical Directors regarding resident care and facility practice. Communicates with the Chief Clinical Officer and Operations Resource Team to assure coordination of clinical services/consultation to facilities. Monitors facility incorporation of company QAPI and assures appropriate action plan based on QAPI survey outcomes. Attends facility QAPI Committee meetings and related quality improvement committee meetings, evaluates overall performance and provides recommendations for improvement. Provides leadership and direction to facility Director of Nursing, Assistant Director of Nursing, Staff Development Coordinator, and MDS Coordinator and assists with coordinating quality improvement processes. Completes timely, quality site visits and prepares timely, detailed written reports identifying key issues.  Monitors facility’s response to site visit reports and consultant reports for timeliness of intervention and evaluation of effectiveness.Participates in and assists with facility NHA with pre-survey preparation, surveys, complaint investigations and occurrences as assigned. Assists with education of facility nursing staff in the principles and expectations related to residents’ rights, dignity and confidentiality.Adheres to corporate policies and procedures. Educates facility staff on current health care issues and new industry developments. Embraces continual forward progression of culture change and incorporates the Eden principles consistently with regard to all decisions and recommendations both at the facility and at the home office. Utilizes interpersonal communication skills, conflict management skills and leadership skills, including mentoring and coaching. Completes all appropriate paperwork and reports accurately and timely.Participates in various facility and corporate committees and task forces.Participates in outside committees and associations to represent the management company in the industry and to gain updated information on trends and activity within the health care industry.Accepts clinical management position assignments at managed facilities and functions within the job description for these assigned positions as needed (i.e., Director of Nursing).Assists with the development, education and implementation of new policies and procedures as needed. Communicates policy and procedure changes to facility personnel and team members to maintain awareness. Remains current on new federal, state and local nursing regulations as they apply to long-term care operations.Works with facility management to develop nursing budgets. Assists in marketing efforts for census development.Assists in the development of job descriptions for team members and facility positions.Some overnight stays may be required.Perform all other duties, as requested. THE MUST HAVES:Must possess, as a minimum, a Masters of Social Work degree from an accredited college or universityLicensed Clinical Social Worker (LCSW)  currently licensed in the state of Colorado preferred. Licensed Social Worker (LSW) with Long Term Care experience and working on completion of clinical hours may be consideredMust have, as a minimum, two (2) years of experience as a social worker in Long Term Care. Must have experience working with the psychiatric populationExperience in a healthcare consultant role preferred    Must have a valid, unrestricted Colorado driver’s licenseAbility to pass a criminal background check as well as Colorado CAPS background check.COVID Vaccine Required or Valid Exemption.Vivage is  an Equal Opportunity Employer
