The Physical Nutritional Management Team Nurse, under the direction of the Director of Habilitation Therapies (DHT), is responsible for actively serving as a dedicated core team member of the Physical Nutritional Management Team (PNMT), a team of specialists with knowledge and expertise in the development of Physical Nutritional Management Plans who meet to provide comprehensive assessment and determine appropriate intervention for persons whose identified health status places them at the highest risk of potential or actual injury and/or illness. The PNMT nurse along with the team will perform specialized assessments, analyze the results of assessments, make recommendations with measurable outcomes, develop action plans, determine monitors and monitoring schedules, re-evaluate regularly as determined by risk level and outcomes, and integrate recommendations into the Personal Support Plan (PSP). The position requires a working knowledge of the Settlement Agreement Section O (Minimum Common Elements of Physical Nutritional Management), Health Care Guidelines for Physical Management, State Supported Living Centers At-Risk policy and process, nursing issues in Physical Nutritional Management including but not limited to enteral eating, dysphagia, aspiration, medication administration monitoring, pneumonia/respiratory problems, dehydration, hypothermia, seizures, urinary tract infections. The PNMT nurse shall demonstrate clear and concise verbal and written English communication skills; computer skills; and the ability to facilitate discussions/meetings and work collaboratively with the Personal Support Team (PST), medical staff, and other disciplines. The nurse is under the direction of the DHT with considerable latitude for the use of initiative and independent judgment. The position will provide regular, thorough analysis of the PNMT process/operations with recommendations for improvement to the DHT. In addition, this position will provide training and guidance to nursing and other staff as it relates to physical nutritional management. Provide treatment to HHS employees injured in course and scope of employment. Essential Job Functions: Attends work on a regular basis and may be required to work a specific shift schedule or, at times, even a rotating schedule, extended shift and/or overtime in accordance with agency leave policy and performs other duties as assigned. Performs assessments of individuals who are determined to be a highest risk and who have been referred to the PNMT. Participates in design of programming and supports based on assessment. Performs competency-based training for staff in working with individuals who are at high risk. Participates as an active member of the Physical Nutritional Management Team and PST for individuals served. Monitors PNMT recommendations, implementation, outcomes, documentation and other activities related to PNMT activities. Must demonstrate considerable knowledge of nursing techniques and procedures; of health care laws and regulations; of medical diagnoses and procedures; of accepted medical treatment patterns; of community health and nursing care principles, practices, and procedures; of program regulations and procedures. Medical record interpretation, including diagnosis and treatment planning. Project facilitation. Analysis of data, making and implementing recommendations. Provide treatment to HHS employees injured in course and scope of employment. Other duties as assigned include but are not limited to actively participating and/or serving in a supporting role to meet the agency's obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation. Such participation may require an alternate shift pattern assignment and/or location. Knowledge Skills Abilities: Knowledge of nursing techniques and procedures; of health care laws and regulations; specialized training or experience demonstrating competence in working with individuals with complex physical and nutritional management needs; medical diagnoses and procedures; accepted medical treatment practices; program regulations and procedures; and quality management tracking and trending. Working knowledge of evidence-based practices applicable to treatment of individuals with developmental disabilities. Knowledge of Root Cause Analysis. Must have the ability to: provide patient care; prepare and maintain records and databases; interpret policies and procedures; interpret health care regulations; appropriately interact with individuals with developmental disabilities and staff members. Knowledge of nursing care standards and best practices. Ability to interpret regulations and communicate them effectively both orally and in writing. Knowledge of state and federal laws and ICF-IID rules, regulations, policy and procedures related to State Supported Living Center campuses preferred. Strong knowledge and application of computer/software. Strong interpersonal skills with the ability to negotiate and facilitate. Ability to work independently and productively with all levels of staff. Ability to prioritize work in alignment with departmental needs and manage multiple deadlines. Ability to motivate all levels of staff and facilitate change. Proven ability to demonstrate flexibility and willingness to accept additional responsibility as needed. Ability to make nursing decisions and recommendations to the team. Physical skills to use CPR, restraint and Ukeru techniques (once trained). Registration or Licensure Requirements: Licensed to practice as a registered nurse in the State of Texas (or a state that recognizes reciprocity through the Nurse Licensure Compact) Initial Selection Criteria: Six (6) months experience working as a practicing registered nurse is required. Two years is preferred. At least one (1) years of experience as a practicing registered nurse in a health agency, hospital, or health-care facility is preferred. Bachelor's Degree is preferred. Experience in assessment and documentation of health status is required. Males between the ages of 18 - 25 must be registered with the Selective Service. All State Supported Living Center employees are subject to Random drug testing. Flexibility in work hours may be required for this position. The position may be required to work overtime and/or extended hours. Must be willing to work weekends and holidays.
