Under the direction of the Central Nurse Triage Manager, the Central Triage Site Ambassador will foster a positive collaborative relationship with all Physician offices supported by the Centralized Nurse Triage Team. This role will perform all duties and responsibilities of a Phone Triage nurse while working collaboratively, both in person and remotely, with the Physician's office staff. This role will identify workflow challenges and promote increased collaboration between Central Nurse Triage and the Physician office staff. The Central Triage Site Ambassador will address patient concerns, prioritize responses, and escalate urgent and time-sensitive situations to a physician, Physician's Assistant, or Nurse Practitioner. This role will require on-site support for offices supported by CNT three days per week and will work remotely two days per week. Responsibilities MAJOR RESPONSIBILITIES Manage patient health concerns and symptoms via telephone interactions * Utilize physician-approved EPIC protocols for the assessment and management of patient care needs * Assess patient's medical condition and provide medical advice based on Duly protocols and Provider directives * Order tests, schedule procedures and appointments based on patient need * Responds to, and prioritizes, incoming patient messages and test results * Delivers patient education, according to the individualized needs of the patient and as prescribed by the Provider. * Educates patient and/or family members about diagnostic procedures, medications, nutrition and health and wellness maintenance. * Responds to patient questions, and appropriately provides reassurance while responding to patient concerns Partners with the sites supported by Centralized Nurse Triage to promote increased cooperation and communication between the sites and triage department * Collaborate with the Physician office staff while providing patient care and advice * Encourage open and honest communication between CNT and Physician partner offices * Escalate any concerns that may arise to Central Nurse Triage Manager * Receive, and communicate, recommendations for improvements based on interactions with Physician partners in the office * Respond to and implement protocol changes based on departmental needs * Work onsite in Physician clinical office three days per week to cultivate relationships and promote collaboration Maintains accurate and detailed patient records and pertinent information * Document patient care notes for all calls into the Medical Record * Document and communicate patient education that has been provided Performs and delegates a variety of tasks to facilitate patient care * Serve as a peer resource for CNT questions and workflows * Provides patient care via phone interaction, focusing on the needs of the patient while exemplifying service excellence, care, concern, respect, and cooperation * Serves as a patient advocate by being the initial point of contact for patient-related issues * Focus on one touch nursing with a goal of taking care of all patient concerns on the first encounter Qualifications MINIMUM EDUCATION AND EXPERIENCE REQUIRED License / Registration / Certification Illinois RN License Issued by (Governing Body) State of Illinois Level of Education RN (required) BA (preferred) Field of Study (if applicable) Nursing Years of Experience 3-5 years' RN experience (required) Describe Type of Experience One year of Professional Nursing in a clinical or hospital setting (preferred) MINIMUM KNOWLEDGE, SKILLS, AND ABILITIES * Must possess excellent written and verbal communications skills * Able to communicate and collaborate with all levels of the organization * Able to anticipate and appropriately respond to patient care needs * Able to work independently and as a member of a team * Able to prioritize tasks based on departmental need * Able to adapt care, recommendations and patient education based on age and culture as well as any other patient specific needs and preferences * Knowledge of HIPAA privacy policy Monday- Friday 8:30-5, two days from home, three days working onsite at the Oak Lawn office.
