Job Title: Registered Nurse

VNA Health Group is seeking a skilled Registered Nurse to join our team in providing high-quality home health care services to patients in Brick, New Jersey.

Job Summary:

The ideal candidate will have a strong background in nursing, excellent communication skills, and the ability to work independently in a fast-paced environment.

Responsibilities:

• Assess and evaluate patient needs, developing individualized care plans to ensure optimal outcomes.
• Provide education and support to patients and families, promoting health literacy and self-management skills.
• Collaborate with interdisciplinary teams to coordinate care and ensure seamless transitions.
• Document patient progress, updating care plans and outcomes as needed.

Requirements:

• RN with Bachelor's degree in Nursing, preferred.
• 1 year of supervised clinical experience as a registered nurse.
• Excellent verbal and written communication skills.
• Strong computer skills and proficiency in electronic medical records.
• Excellent organizational and interpersonal skills.
• Valid driver's license and reliable transportation.
