The primary purpose of the VOARRC Case Manager role is to provide specialist, end-to-end case management of residents, with the aim of supporting the resident to take responsibility for completing activities aimed at addressing their rehabilitation and reintegration needs and ultimately to reduce the likelihood and seriousness of re-offending. The Case Manager will operate within case management frameworks which are aligned to reducing re-offending operating principles and frameworks for rehabilitation and reintegration.

The role includes all facets of case management from initial assessment, identification of resident needs and case planning incorporating specialist rehabilitation and reintegration interventions and services, as well as facilitating links with the resident's support groups and external providers.

Central to the role will be the ability to exercise sound professional judgement to ensure that residents' needs and services are appropriately identified and provided and that plan monitoring, review and management is aligned to program and resident need.

Job Responsibilities

Case Management:

  • Develop an enhanced reintegration plan.
  • Proactively remain informed on resident progress and events.
  • Encourage resident motivation through the use of a range of techniques and tools including evidence-based practice, motivational interviewing.
  • Monitor resident progress against the plan and regularly review resident plans ensuring all rehabilitation and reintegration interventions and services are provided to the resident according to the standards set for standard and enhanced case management.
  • Develop and maintain effective networks to assist with reintegration of residents.
  • Ensure the reintegration needs of residents are met prior to discharge to the community, including preparation of individual reentry plans in consultation with service providers.
  • Liaise with BOP, DOC Probation Officers on a regular basis to facilitate "seamless" transitions to the community.
  • Provide timely support and advice to Program Service Providers.
  • Assist in completion of risk and needs assessment of residents including identification of needs and priorities and recommending residents for either standard or enhanced case management as recommended by the assessment instrument.

Monitoring and Reporting:

Keep timely and accurate case notes for each resident for inclusion on their personal file.

  • Ensure that residents have the required reports and support and ensure that the reports are provided in a timely manner and meet the required standard.

Health & Safety:

  • Ensure that all work is carried out in a safe and responsible manner that does not compromise your own or the health and safety of others in the workplace.
  • Comply with VOA's Standards of Conduct and policy statements on all matters of Occupational Health and Safety.

Person Specification

Knowledge, Skills and Experience:

  • Knowledge of and experience in case management and/or rehabilitation and reintegration disciplines and practices.
  • Knowledge of the factors impacting on offender rehabilitation and reintegration.
  • Effective relationship management skills, including the ability to encourage and lead co-operative efforts and identify and manage potential areas of conflict.
  • Ability to make sound professional judgements after considering standards, policies and procedures and tailoring solutions to individual circumstances.
  • Effective communication skills, including being able to communicate verbally in a respectful and non-threatening manner that is tailored to the audience
  • Effective written skills including the ability to write clear and meaningful reports for different audiences.

Experience and Qualifications

  • A Baccalaureate degree or equivalent experience in criminal justice and experience with chemical dependency/mental health populations.
  • Counseling Certification preferred.
  • A minimum of 2-years experience in corrections or related field is desired.
  • Excellent communication and writing skills.
