About Care Crisis Response Services (CCRS):
CCRS is a 24/7 crisis center that provides suicide prevention/intervention, crisis intervention, and professional consultation services, including triaging requests for behavioral health evaluations. We provide brief crisis counseling and assist the community and clients in determining what services are most appropriate for the current behavioral health crisis. We operate regional and 988 crisis lines throughout the state of Washington. General Function:
This full-time, salaried position works as part of a crisis line team to assist in providing crisis care coordination in accordance with a plan of care that meets identified client needs and incorporates services/resources to improve outcomes. The care coordinator works with clients to provide information and guidance for effective ongoing crisis management. Care coordination is conducted through telephonic communication with clients and/or their caregivers. The work is focused on selected populations who utilize crisis services.
Principal Activities:
- · Schedule urgent, next day appointments and conduct post appointment follow-up by phone
- · Call clients who miss appointments to check-in and address barriers
- · Complete best practice safety planning
- · Develop care plans including crisis plans
- · Collaborate with other providers to coordinate care
- · Bridge communication between clients, internal counseling staff, and external professionals
- · Keep clients engaged in care through collaborative goal setting and tracking
- · Link clients to community resources identified to address gaps in the healthcare continuum
- · Check-in regularly with clients to evaluate progress
- · Document all client contacts in the electronic health record
- · Other duties as assigned

Qualifications
Qualifications:
- · Bachelor's degree in social work, psychology, human services, or related field and one year of related work experience in a mental health setting
- · Work in case management or social work is preferred
- · Eligibility for Agency Affiliated Counselor prior to date of hire and credential obtained with 60 days of hire
- · Computer skills and proficiency in Microsoft Office
- · Strong analytical thinking and the ability to handle multiple tasks concurrently
- · A creative mind with an ability to suggest improvements
- · Willingness to take on extra tasks as needed
- · Experience working with diverse populations
- · Knowledge of and/or willingness to learn trauma informed practices
