This position provides quality care to consumers in an environment, which promotes their rights, dignity, individuality, and needs. Implement specific procedures and programs; coordinate work within the department and service site; report pertinent information to the immediate supervisor; respond to inquiries or requests for information; assist the immediate supervisor with tasks to support the Department.

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.

ESSENTIAL FUNCTIONS

- Assist by supporting the division of shift responsibilities and tasks to ensure proper care of consumers.

- Assist with up to two hours of administrative tasks per week as delegated by Supported Living Supervisor.

- Be available and willing to cover for call offs and open shifts as needed.

- Audit books and paperwork each shift and follow up with staff to make corrections as needed.

- Maintain trainings and keep an attitude of flexibility to help at all houses in the Senior Program Manager's caseload.

- Help Supported Living Supervisor with on-call responsibilities for vacations, leaves, etc.

- Participate in on-the-job training and other ongoing training to ensure job knowledge and capabilities are current.

- Provide input and feedback to supervisor regarding employee issues as requested and needed.

- Be available to cover shifts as needed in support of the service site's needs.

- Ensure complete implementation of Individualized Support Plan (ISP), community activities, and other tasks at the site, to maintain the highest quality of service.

- Assist consumers with their finances, shopping and bill paying and other daily requirements as needed.

- Maintain professional standards including following company policies related to attendance, punctuality, documentation, dress code and all other company expectations.

- Complete all required documentation in a timely manner, including billing documentation submission.

- Provide assistance to those we serve for their medical needs, including medication administration, and timely reporting of any medical issues/concerns.

- Implement high needs plans and/or behavior plans as outlined by the immediate supervisor and interdisciplinary team.

- Ensure community activities are occurring as planned by providing transportation and care to those we serve for the various activities.

- Maintain professional standards including following company policies related to attendance, punctuality, documentation, dress code and all other company expectations.

- Provide individualized attention to those we serve, which encourages each individual's ability to maintain or attain the highest practical physical and mental well-being.

- Promote health and safety of all individuals and staff through adherence to all Wabash Center and Departmental policies and procedures and protect consumers from abuse/neglect.

- Maintain all appropriate information in a confidential and professional manner, and in accordance with state and federal rules and regulations.

- Ensure all verbal and written communications are presented in a professional manner, reflecting dignity and respect for those we serve, supervisors, and other co-workers.

- Communicate with those we serve and their families, guardians, case managers, behaviorist, day programs, and any other individuals or entities associated with the individuals in a manner that promotes positive interaction and understanding.

- Actively participate in department and/or service site discussions and meetings as needed and take action on all assigned items.

- Immediately alert supervisor/manager of any potentially critical issue or crisis situation.

- Uses emergency on call system appropriately.

- Assist the immediate supervisor with various administrative tasks to support department operations as directed; may also be assigned special projects.

- Perform tasks which are supportive in nature to the essential functions of the job, but which may be altered or redesigned depending upon an individual's circumstances and/or Wabash Center and regulatory agency policy changes.

- Meet on-going position requirements including training.

QUALIFICATIONS

- High school diploma/GED or other relevant experience preferred.

- Experience in direct support service and/or working with intellectual disabilities preferred.

- Ability to learn and effectively utilize computer software applications to accomplish tasks.

- Ability to read, comprehend, interpret and apply information from general procedures, instructions, data and

- Ability to learn new concepts, which will be provided through departmental training sessions on an ongoing basis.

- Must provide own transportation, have a valid driver's license, registration, and proof of acceptable driving record.

PHYSICAL REQUIREMENT

- Must have the ability to lift/move individuals and/or materials weighing up to 75 lbs. occasionally.

- Must be able to complete fit for duty evaluation based on the individual position where required.

- Must have the ability to stand, walk, bend, stoop, kneel and reach frequently as needed to serve individuals that we serve.

Job Type: Full-time

Pay: From $15.00 per hour
