What are the responsibilities and job description for the SOCIAL WORKER-HOME AND COMMUNITY BASED SERVICES position at Wabasha County?
- Assist client in obtaining/maintaining an optimal functioning level within the community, and living in the least restrictive setting possible
- Conduct eligibility screenings following all current DHS timelines and procedures
- Conduct mandated client visits and reviews with client within established timeframes
- Develop Individual Service Plan or Community Support Plan in conjunction with client/family/guardian, within required timelines; monitor provider compliance with plan
- Arrange for or coordinate services as outlines in plan; adopt plan; and respond to client change in need in a flexible manner; handle and respond to crisis situations as needed
- Insure that all necessary forms for public assistance are completed in a timely fashion
- Initiate and follow-through on Guardian/Conservator proceedings as needed
- Additional possible tasks include: review, evaluate and respond to requests for host county concurrence; Family Support Grant, conduct screenings for nursing facility admission
- Follow all current guidelines or requirement for Waiver Case Management or Targeted Case Management reimbursement
