Wafa House is a nonprofit organization that specializes in serving the needs of survivors of domestic violence. This organization offers a range of services with a culturally sensitive approach including clinical mental health services, social services, and legal advocacy and representation. At Wafa House, our mission is to serve and strengthen individuals who need financial and emotional support and help them feel a sense of community.
Purpose: At Wafa House, a case manager will be responsible for completing the four essential levels of case management: intakes/referrals, needs assessment, service planning, and monitoring with situational evaluation. Case managers should be able to maintain a caseload of clients and advocate for them on a regular basis. They would complete a detailed intake form for each client and create a care plan assessing the needs of the client. They will monitor the client, and evaluate their situations, and develop connections within social services.
Duties and Responsibilities
Case Management
- Complete a detailed client intake form
- Facilitate patient care by assessing individual needs
- Creating individualized treatment plans
- Maintain confidential client records and reports on a timely basis
- Apricot service tracking (progress notes on each client contact)
- Network with community agencies, service providers, etc.
- Provide client advocacy and find resources for clients as needed
- Create meaningful relationships with clients for rapport-building
- Coordinating the client's treatment program by making referrals and communicating with relevant collaterals.
- Meeting with clients on a regular basis
- Establishing plans for clients
Miscellaneous
- Perform other duties as assigned

Qualifications
- Must relate well to clients in a caring, professional, and unbiased manner
- Must be fluent in Arabic
- Bachelor's degree in Social Work or related field (MSW is preferred)
- Effective verbal and written communication
- Strong and defined sense of professional boundaries
- Effective analytical and problem-solving skills
- Ability to exercise sound judgment and decision-making
- Successful in working collaboratively and harmoniously with the Wafa House team
- Excellent time management and prioritization skills
- Ability to self-govern and work with minimal supervision
- Proven ability to interact efficiently and communicate effectively with people from diverse backgrounds
